带血管蒂腓骨移植修复下肢长段骨缺损的长期随访观察 被引量:4

Long Term Follow-up of Fibular Graft with Vessel Pedicle in Repairing Long Segmental Bone Deficits of Lower Limbs

摘要 目的:探讨腓骨移植修复下肢长段骨缺损的疗效。方法:采用带血管蒂腓骨移植修复下肢长段骨缺损18例,其中双腓骨4例,单腓骨14例、外固定架固定12例,接骨板固定6例。结果:所有患者术后随访2~8a,平均4.4a。移植骨2~6个月(平均3.4个月)愈合。移植骨均有不同程度增粗表现,其中12例使用外固定架固定的患者移植骨增粗更为明显。结论:采用带血管蒂腓骨移植修复下肢长段骨缺损,远期随访效果满意。采用多平面外固定架固定,掌握好负重行走的时机和负荷量可促使移植骨愈合并增粗,逐渐达到受区骨直径。 Objective:To discuss the curative effects of fibular graft with vessel pedicle in repairing long segmental bone deficits of lower limbs. Methods:Fibular grafts with vessel pedicle were used in repairing long segmental bone deficit of lower limb for 18 cases, double-barreled fibular graft 4 cases and single fibular graft 14 cases. Twelve cases were fixed by external fixation devices, 6 cases were fixed by plates. Results:All the cases were followed up for 4.4 a averagely ( range 2 - 8 a). The average time for host-autograft union was 3.4 months (range 2 -6 months). All the grafted bone had different level of hypertrophy, especially for the 12 cases fixed by external fixation devices. Conclusions:It was satisfactory to use fibular grafting with vessel pedicle to repair long segmental bone deficits of lower limbs basing on the long term follow-up. Adopting muhiplane external fixation devices and controlling well weight bearing would enhance union and hypertrophy of grafted bone, and its diameter would match the recipient site with time.
