摘要
目的探讨个性化低对比剂用量结合CT智能最佳管电流调节(Care Dose 4D)技术、智能最佳管电压(Care Kv)技术在冠状动脉CTA检查中的应用价值。方法选取2018年1月—2020年1月于本院拟行冠状动脉CTA的120例患者作为研究对象。采用随机数表法将其分为A组、B组、C组三组,每组各40例。其中A组扫描条件采用固定管电压120 kV以及Care Dose 4D技术,B组、C组扫描条件则采用Care Dose 4D技术与Care Kv技术,并且A组、C组对比剂剂量为0.8 ml/kg,流速控制5 m/s,B组则根据患者体重个性化调整剂量,对比三组冠状动脉CTA成像质量及所受辐射剂量情况。结果三组患者在平均体重上的比较,差异均无统计学意义(P>0.05);B组患者使用对比剂用量上明显低于A组、C组,差异具有统计学意义(P<0.05);B组、C组患者在CTDIvol、ED上均明显低于A组,差异具有统计学意义(P<0.05);三组患者CTA扫描模式下图像质量参数比较,差异均无统计学意义(P>0.05);与金标准DSA比较,三组患者CTA准确率均较高,且差异均无统计学意义(P>0.05)。结论在冠状动脉CTA检查中采用智能低辐射剂量技术与个性化低对比剂用量可在保证图像质量与诊断效能的前提下,降低患者所受辐射剂量以及对比剂对机体的不良影响,进一步提升冠状动脉CTA检查的安全性与可靠性,具有临床应用及推广价值。
Objective To explore the application value of personalized low-contrast agent dosage combined with CT intelligent Care Dose 4D technology and intelligent Care Kv technology in coronary CTA examination.Methods 120 patients those who planned to undergo coronary CTA in our hospital from January 2018 to January 2020 were selected as the research subjects.They were divided into three groups,including group A,B,and C according to random number table method,40 cases in each group.Patients in group A used fixed tube voltage of 120Kv and Care Dose 4D technology,the 40 cases in groups B and C used Care Dose 4D technology and intelligent Care Kv technology.The dose of contrast agent in group A and C was 0.8ml/kg,the flow rate was 5m/s,the dose in group B was adjusted according to the patient's weight,the imaging quality and radiation dose of coronary CTA in the three groups were compared.Results There was no significant difference in the average body weight among the three groups of patients(P>0.05).Contrast dosage was significantly lower in group B than in group A and C(P<0.05).CTDIvol and ED indicators in groups B and C were significantly lower than those in group A(P<0.05).There were no significant differences in the image quality parameters among the three groups in CTA scanning modes(P>0.05).Compared with the gold standard DSA,there was no significant difference in the accuracy among the three groups in CTA(P>0.05).Conclusions The use of intelligent lowradiation dose technology and personalized low-contrast agent dosage in coronary CTA examination could reduce the radiation dose of patients and the adverse impact on the body induced by contrast agents under the premise of ensuring image quality and diagnostic efficiency,and further improve the safety and reliability of coronary CTA examination and have clinical application and promotion value.
