重性抑郁障碍患者言语流畅性任务的脑血流动力学特征研究 被引量:13

摘要 目的基于近红外功能成像技术探索重性抑郁障碍(major depressive disorder,MDD)患者在言语流畅性任务中脑血流动力学反应特征,分析脑血流动力学量化指标在区分MDD中的应用价值。方法纳入53例MDD患者和54名健康对照,采用功能近红外光谱成像仪(ETG-4000)测量两组在言语流畅性任务下前额和双侧颞区血流动力学反应特征,计算任务时脑血流动力学反应强度(积分值)与脑血流动力学反应强度达一半时所需时间(重心值)。结果在言语流畅性任务中,MDD组前额及双侧颞区的血流动力学反应强度低于对照组[前额,(31.43±48.22)mmol·mm vs.(120.28±98.13)mmol·mm;双侧颞区,(72.50±101.55)mmol·mm vs.(147.11±135.93)mmol·mm],差异有统计学意义(均P<0.01)。受试者工作特征曲线分析显示,前额积分值区分MDD组曲线下面积为0.811,前额积分值的最佳截断值(30.45 mmol·mm)下区分MDD的敏感度为56.6%,特异度为88.9%。结论MDD患者在进行言语流畅性任务时前额及双侧颞区血流动力学反应强度低于对照,前额积分值对区分MDD具有一定价值。 Objective This study aims to explore the characteristics of cerebral hemodynamic response in major depressive disorder(MDD)under verbal fluency task(VFT)and the diagnostic value of quantitative indexes of cerebral hemodynamics in MDD using functional near-infrared spectroscopy imaging(fNIRS).Methods The study included 53 patients with MDD and 54 healthy controls.Characteristics of hemodynamic response in the forehead and bilateral temporal brain areas under VFT were measured by using the Hitachi ETG-4000 fNIRS system.The total intensity of the cerebral hemodynamic response(integral value)and the time required to reach half the intensity of the cerebral hemodynamic response(centroid value)were calculated.Results Hemodynamic response in the forehead and bilateral temporal brain areas under the VFT task of MDD group was lower than that of the control group[(31.43±48.22)vs.(120.28±98.13)mmol·mm;(72.50±101.55)vs.(147.11±135.93)mmol·mm,P<0.01].The forehead integral value indicated a certain diagnostic value for the diagnosis of MDD with the area under the ROC curve 0.811.The optimal cut-off value of the forehead integral value was 30.45 mmol·mm.The sensitivity of forehead integral value was 56.6% and the specificity was 88.9%in diagnosing MDD.Conclusion The intensity of hemodynamic response in the prefrontal and bilateral temporal regions was lower in MDD group than in healthy controls under VFT.The forehead integral value has certain value in the diagnosis of MDD.
