摘要
目的:探讨胬肉切除术中应用角膜缘干细胞结膜瓣移植对患者泪膜功能及并发症的影响。方法:选取2017年1月至2019年1月陕西省咸阳市第一人民医院收治的翼状胬肉患者120例,按随机数字表法分为干细胞组(60例)和结膜瓣组(60例),结膜瓣组行胬肉切除术+结膜瓣移植,干细胞组行胬肉切除术+角膜缘干细胞结膜瓣移植治疗,比较两组术后泪膜功能[Shirmer泪液分泌试验(SIT)、泪膜破裂时间(BUT)]、裸眼视力(UCVA)、并发症、复发、生活质量[眼表疾病指数(OSDI)]情况。结果:两组术后3个、6个、12个月SIT、BUT、UCVA均明显高于术前(均P<0.05),且同一时点干细胞组均明显高于结膜瓣组(均P<0.05);两组术后3个、6个、12个月OSDI评分均明显低于术前(均P<0.05),且同一时点干细胞组OSDI评分均明显低于结膜瓣组(均P<0.05)。干细胞组并发症发生率和复发率均明显低于结膜瓣组(均P<0.05)。结论:胬肉切除术中应用角膜缘干细胞结膜瓣移植可有效改善患者泪膜功能及视力,且可减少并发症及复发,有利于提高生活质量,值得临床推广。
Objective To discuss the effect of limbal stem cell conjunctival flap transplantation on the tear film function and complications in pterygium resection.Methods:From January 2017 to January 2019,120 patients with pterygium admitted to Xianyang City First People's Hospital were selected.They were randomly divided into the stem cell group(n=60)and the conjunctival flap group(n=60).The conjunctival flap group was treated with pterygium resection+conjunctival flap transplantation,the stem cell group was treated with pterygium resection+limbal stem cell conjunctival flap transplantation.The tear film function[shirmer tear secretion test(SIT),break up time(BUT)],uncorrected visual acuity(UCVA),complications,recurrence,and quality of life[ocular surface disease index(OSDI)]were compared between the two groups.Results:The SIT,BUT,and UCVA in the two groups at 3,6,and 12 months after operation were significantly higher than before operation(P<0.05),and the SIT,BUT,and UCVA in the stem cell group were significantly higher than the conjunctival flap group(P<0.05).The OSDI score in the two groups at 3,6,and 12 months after operation were significantly lower than before operation(P<0.05),and the OSDI score in the stem cell group were significantly lower than the conjunctival flap group(P<0.05).The incidence of complications and recurrence rate in the stem cell group were significantly lower than the conjunctival flap group(P<0.05).Conclusion:Limbal stem cell conjunctival flap transplantation can effectively improve the tear film function and vision of patients,and it can reduce complications and recurrence.It is helpful to improve quality of life and is worthy of clinical promotion.
