摘要
目的研究术中不同保温方案对全麻腹腔镜直肠癌术后患者麻醉复苏及应激反应的影响。方法选取我院收治的全麻腹腔镜直肠癌术后患者125例,按照随机数字表法分为对照组(62例)和观察组(63例)。对照组采用棉被覆盖加输注常温液体进行被动保温,观察组给予医用升温毯和输注保温液体进行主动保温。观察两组患者体温稳定、麻醉复苏、应激反应情况。结果术中30 min、术后即刻,观察组肛温均显著高于对照组(P<0.05);观察组苏醒时间显著短于对照组(P<0.05),苏醒期寒战发生率显著低于对照组(P<0.05),苏醒期躁动评分低于对照组(P<0.05);两组苏醒期躁动发生率比较,P>0.05;术后即刻,两组C反应蛋白(CRP)、肾上腺激素(AD)、去甲肾上腺素(NE)均显著升高(P<0.05),但观察组显著低于对照组(P<0.05)。结论医用升温毯和输注保温液体的主动保温方案应用于全麻腹腔镜直肠癌术后患者可通过维持体温稳定来促进麻醉复苏、降低应激反应。
Objective To explore the effect of different intraoperative thermal insulation methods on anesthesia recov⁃ery and stress response of patients after laparoscopic rectal cancer surgery with general anesthesia.Methods 125 patients sfter laparoscopic rectal cancer surgery with general anesthesia were selected and randomly divided into the control group(62 cases)and the observation group(63 cases).The control group was covered with cotton quilt and injected with normal temper⁃ature liquid,while the observation group was given medical heating blanket and injected with thermal insulation liquid.Tem⁃perature stability,anesthesia recovery and stress response of the two groups were observed.Results The anal temperature of the observation group was significantly higher than that of the control group at 30 min during operation and immediately after operation(P<0.05).The waking time of the observation group was significantly shorter than that of the control group(P<0.05);the incidence of chills during waking period was significantly lower than that of the control group(P<0.05);the score of restlessness during waking period was significantly lower than that of the control group(P<0.05).There was no statistically significant difference between the two groups in the incidence of restlessness during the wake period(P>0.05).Immediately af⁃ter the operation,the c-reactive protein(CRP),adrenal hormone(AD)and norepinephrine(NE)in the two groups were signifi⁃cantly increased(P<0.05),but those in the observation group were significantly lower than those in the control group(P<0.05).Conclusion The active thermal insulation of medical heating blanket and infusion of thermal insulation liquid applied to patients after laparoscopic rectal cancer surgery with general anesthesia can promote anesthesia recovery and reduce stress response by maintaining the stability of body temperature.
