摘要
目的探究基于临终关怀模式的情绪管理对老年晚期肺癌化疗患者癌因性疲乏、希望水平、心理健康状况及免疫功能的影响。方法选取我院收治的老年晚期肺癌化疗患者135例,随机数字表法分为两组。两组均给予常规肿瘤内科化疗,对照组67例给予正念减压疗法,观察组68例给予基于临终关怀模式的情绪管理,对比两组患者1个月末癌因性疲乏与希望水平:采用Piper疲乏修订量表(RPFS)评估两组患者的心理健康状况和欧洲癌症研究生存质量核心问卷(EORTC QLQ-C30)等工具对比两组免疫功能:CD3+、CD4+、CD8+和NK细胞水平。结果观察组RPFS、HAMD、HAMA评分,CD3+、CD4+、CD8+、NK水平低于对照组,EORTC QLQ-C30评分高于对照组(P<0.05)。结论基于临终关怀模式的情绪管理可改善老年晚期肺癌化疗患者的焦虑、抑郁情绪,降低癌因性疲乏症状,提高免疫功能和希望水平。
Objective To explore the effect of emotional management based on hospice care model on cancer-relat⁃ed fatigue,hope level and immune function of elderly patients with advanced lung cancer chemotherapy.Methods 135 el⁃derly patients with advanced lung cancer chemotherapy were selected as the research objects and randomly divided into the observation group and the control group.Both groups were given conventional chemotherapy.67 patients in the control group were given mindfulness decompression therapy,and 68 patients in the observation group were given emotional man⁃agement based on the hospice care model.Piper fatigue revision Scale(RPFS),Hamilton Depression Scale(HAMD),Ham⁃ilton Anxiety Scale(HAMA)and European Cancer Research Quality of Life Core Questionnaire(EORTC QLQ-C30)and other tools were used to compare cancer-related fatigue and hope level of the two groups at the end of 1 month;the im⁃mune function(CD3+,CD4+,CD8+)and NK cell percentage of the two groups were compared.Results The RPFS,HAMD,HAMA scores,CD3+,CD4+,CD8+and NK levels in the observation group were lower than those in the control group;the EORTC QLQ-C30 score was higher than that in the control group(P<0.05).Conclusion Emotional management based on hospice care model can improve anxiety and depression in elderly patients with advanced lung cancer chemotherapy,re⁃duce symptoms of cancer-induced fatigue and improve immunity and hope level.
