摘要
目的分析东莞西南区人乳头瘤病毒(Human papillomavirus,HPV)-DNA检测及液基薄层细胞检测(thinprepcytologic,TCT)在宫颈癌疾病筛查中的应用价值。方法方便选取2018年1月-2019年12月该院接收的120例宫颈癌筛查妇女纳入至该实验中,所选人员均接受宫颈病理活检,同时开展HPV-DNA与TCT检测,记录病理检测结果、HPV-DNA检测结果、TCT检测结果、TCT检测与HPV-DNA检查特异度、灵敏度、阴性预测值和阳性预测值进行分析。结果120例受检人员,经病理检查,阴性、阳性分别有75例和45例。经HPV-DNA检测,阳性117例、3例阴性;经TCT检测,阳性10例,阴性110例。HPV-DNA、TCT灵敏度分别是43/45(95.56%)和6/45(13.33%),差异有统计学意义(χ^2=61.329,P<0.05);HPV-DNA与TCT特异度分别是1/75(1.33%)和71/75(94.67%),差异有统计学意义(χ^2=130.876,P<0.05);HPV-DNA和TCT阳性预测值分别是43/117(36.75%)和6/10(60.00%),差异有统计学意义(χ^2=17.518,P<0.05);TCT阴性预测值是71/110(64.55%)。HPV-DNA与TCT检测准确度分别是44/120(36.67%)和77/120(64.17%),差异有统计学意义(χ^2=18.151,P<0.05)。结论宫颈癌筛查采取HPV-DNA检测与TCT检测,每种检测方式具有各自优点,检测期间,联合应用可为筛查工作的开展提供数据参考。
Objective To analyze the application value of human papillomavirus(HPV)-DNA detection and liquid-based cell detection(thinprepcytologic,TCT)in cervical cancer screening in southwestern Dongguan.Methods 120 cases of cervical cancer screening women received in the hospital from January 2018 to December 2019 were conveniently included in this experiment.The selected persons all received cervical pathological biopsy,and HPV-DNA and TCT tests were carried out at the same time,and record the pathological test results,HPV-DNA test results,TCT test results,TCT test and HPV-DNA test specificity,sensitivity,negative predictive value and positive predictive value were analyzed.Results In 120 subjects,the pathological examination showed that 75 cases were negative and 45 cases were positive.After HPV-DNA detection,117 cases were positive and 3 cases were negative;by TCT detection,10 cases were positive,110 cases were negative.The sensitivity of HPV-DNA and TCT are 43/45(95.56%)and 6/45(13.33%)respectively,and the difference was statistically significant(χ^2=61.329,P<0.05);the specificity of HPV-DNA and TCT was 1/75(1.33%)and 71/75(94.67%),and the difference was statistically significant(χ^2=130.876,P<0.05);the positive predictive value of HPV-DNA and TCT were43/117(36.75%)and 6/10(60.00%),and the difference was statistically significant(χ^2=17.518,P<0.05);the negative predictive value of TCT was 71/110(64.55%).The detection accuracy of HPV-DNA and TCT were 44/120(36.67%)and 77/120(64.17%),respectively,and the difference was statistically significant(χ^2=18.151,P<0.05).Conclusion Cervical cancer screening adopts HPV-DNA test and TCT test.Each test method has its own advantages.During the test period,the combined application can provide data reference for the screening work.
