摘要
目的探讨二甲双胍结合吡格列酮对早期糖尿病肾病患者尿白蛋白排泄率(UAER)、同型半胱氨酸(Hcy)水平与肾功能的影响。方法选取2014年1月~2019年12月鹰潭市人民医院收治的60例早期糖尿病肾病患者作为研究对象,按照随机数字表法分为对照组(30例)与观察组(30例)。对照组患者采用吡格列酮治疗,观察组患者采用吡格列酮+二甲双胍治疗,比较两组患者的治疗总有效率、UAER、Hcy水平与肾功能。结果治疗前,两组患者的UAER、Hcy水平和肾功能指标比较,差异无统计学意义(P>0.05);治疗30 d后,观察组患者的总有效率高于对照组,UAER、Hcy水平和肾功能指标水平均低于对照组,差异有统计学意义(P<0.05)。结论二甲双胍结合吡格列酮治疗早期糖尿病肾病的效果确切,能显著降低患者血液中UAER、Hcy水平,减少患者患者肾功能损伤,临床可推广使用。
Objective To investigate the effect of Metformin combined with Pioglitazone on urinary albumin excretion rate(UAER),homocysteine(Hcy)levels and renal function in patients with early diabetic nephropathy.Methods A total of 60 patients with early diabetic nephropathy who were treated in Yingtan People′s Hospital from January 2014 to December 2019 were selected as the subjects,and they were divided into the control group(30 cases)and the observation group(30 cases)according to the random number table method.The control group was treated with Pioglitazone,while observation group was treated with Pioglitazone and Metformin.The total effective rate of treatment,UAER,Hcy level and renal function were compared between the two groups.Results There were no statistical differences in the UAER,Hcy level and renal function indexes between the two groups before treatment(P>0.05).The total effective rate of the observation group was higher than that of the control group after treatment for 30 d,while UAER,the levels of Hcy and renal function indexes in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Metformin combined with Pioglitazone in the treatment of early diabetic nephropathy is effective,it can significantly decrease UAER and Hcy level in blood,and reduce renal function injury,which is worthy of clinical promotion and application.
