中老年人骨密度与H型高血压及糖脂指标的相关分析 被引量:2

Correlation between Bone Mineral Density and H-type Hypertension and Glycolipid Metabolism in Middle-aged and Older People

摘要 目的探究中老年人骨密度(BMD)和H型高血压的关联,以及糖脂指标对BMD的影响。方法采用随机整群抽出东风汽车公司离退休职工体检资料10 804例。记录体检人群的身高、体质量、血压、体质指数(BMI)、腰臀比(WHR),检测甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(GLU)、糖化血红蛋白(HbA1c)、同型半胱氨酸(Hcy)等指标。依据高血压的诊断标准,分为H型高血压组、普通高血压组和正常血压组,对H型高血压与BMD及糖脂代谢指标进行多因素logistic回归分析。结果体检人群骨量降低占比40.7%,骨质疏松占比19.8%;女性明显大于男性,差异有统计学意义(P <0.05)。多因素logistic回归分析显示:以正常血压为参照,年龄、BMI、GLU、TG、HDL-C、Hcy和WHR是普通高血压的保护因素,也是H型高血压的危险因素;以正常骨密度为参照,年龄、BMI、GLU、TG、TC、LDL-C、性别是骨量减少人群的保护因素;年龄、BMI、HDL-C、LDL-C、性别是骨质疏松人群的危险因素;差异均有统计学意义(P <0.05)。H型高血压组的骨量降低和骨质疏松发生率明显高于普通高血压组和正常血压组,差异均有统计学意义(P <0.05)。结论年龄、BMI、HDL-C、LDL-C、性别是老年人骨质疏松的影响因素,年龄、BMI、GLU、TG、HDL-C、Hcy和WHR同时也是H型高血压的危险因素;BMD和H型高血压与糖脂代谢的指标有一定的相关性。 Objective To investigate the relationship between bone mineral density(BMD)and H-type hypertension in middle-aged and older people,and the effect of glycolipid metabolism on BMD.Methods A total of 10804 retired employees from Dongfeng Motor Company who underwent physical examination were selected.The height,weight,blood pressure,body mass index(BMI),waist hip ratio waist-to-hip ratio(WHR),the levels of triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),fasting plasma glucose(FPG),glycosylated hemoglobin(HbAlc)and homocysteine(Hcy)were documented.The patients were divided into H-type hypertension group,ordinary hypertension group,and normal blood pressure group according to the diagnostic criteria of hypertension.Multivariate logistic regression analysis was performed to analyze the correlation between H-type hypertension and BMD and glycolipids metabolism.Results The incidence of osteopenia and osteoporosis was 40.7%and 19.8%respectively,and the proportion of women was significantly higher than that of men(P<0.05).Multivariate logistic regression analysis showed that taking normal blood pressure as reference,age,BMI,FPG,TG,HDL-C,Hcy and WHR were the protective factors of ordinary hypertension and risk factors of H-type hypertension.Taking normal BMD as reference,the age,BMI,FPG,TG,TC,LDL-C,gender were the protective factors of osteopenia.Age,BMI,HDL-C,LDL-C and gender were the risk factors of osteoporosis.There were significant differences between them(P<0.05).The incidence of osteopenia and osteoporosis in H-type hypertension group was significantly higher than that in the ordinary hypertension group and normal blood pressure group(P<0.05).Conclusion Age,BMI,HDL-C,LDL-C and gender may be the influencing factors of osteoporosis in older people.Age,BMI,FPG,TG,HDL-C,Hcy and WHR may be also influential factors of type H hypertension.BMD and H-type hypertension might correlate with the glycolipid metabolism.
