摘要
目的探讨袋鼠式护理对缺氧缺血性脑病新生儿神经系统及体格发育的影响。方法选取2019年1月至2020年3月阳谷县人民医院收治的80例缺氧缺血性脑病新生儿,按住院顺序将其分为对照组和观察组,各40例。对照组给予常规护理方法,包括严格控制室内温度,密切监测生命体征,指导母亲进行母乳喂养、抚触等,遵医嘱严格吸氧、高压氧等常规治疗。观察组在对照组基础上给予袋鼠式护理。比较两组出生40 d后的体格发育水平(身高、头围、体质量增长情况)、护理前后神经发育情况[新生儿行为神经测定(NBNA)标准]以及神经系统损害发生情况。结果观察组的身高、头围和体质量增长水平分别为(4.98±1.28)cm、(2.67±0.99)kg、(7.46±1.73)cm,均高于对照组的(3.36±1.23)cm、(2.06±0.94)kg、(5.93±1.56)cm,差异均有统计学意义(t=5.7717、2.8260、4.1540,P<0.05)。护理前,两组NBNA各项评分比较,差异无统计学意义(P>0.05);护理后,观察组NBNA各项评分明显高于对照组,差异有统计学意义(P<0.05)。观察组神经系统损害发生率显著低于对照组,差异有统计学意义(P<0.05)。结论对缺氧缺血性脑病新生患儿实施袋鼠式护理能有效改善患儿的神经系统损害程度,促进患儿机体发育。
Objective To investigate the effect of kangaroo nursing on nervous system and physical development of neonates with hypoxic-ischemic encephalopathy.Methods From January 2019 to March 2020,80 neonates with hypoxic-ischemic encephalopathy were selected and divided into control group and observation group according to the order of hospitalization,40 cases in each group.The control group was given routine nursing methods,strictly controlled the indoor temperature,closely monitored the vital signs,guided the mother to breast-feeding,touching,etc.,and strictly followed the doctor's advice to take oxygen,hyperbaric oxygen and other routine treatment.The observation group was given kangaroo nursing on the basis of the control group.The physical development level after 40 days of birth(height,head circumference and body mass growth),neurological development before and after nursing(NBNA scores)and the incidence of neurological damage were compared between the two groups.Results The growth levels of height,head circumference and body weight in the observation group were(4.98±1.28)cm,(2.67±0.99)kg,(7.46±1.73)cm,which were higher than those in the control group[(3.36±1.23)cm,(2.06±0.94)kg,(5.93±1.56)cm]respectively,with statistical significance(t=5.7717,2.8260,4.154,P<0.05).Before nursing,there was no significant difference between the observation group and the control group of NBNA scores(P>0.05);after nursing,the NBNA score of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of nervous system damage in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Kangaroo nursing can effectively improve the degree of nervous system damage and promote the development of newborn children with hypoxic-ischemic encephalopathy.
