摘要
目的探讨追踪营养护理方案在食管癌术后放疗患者中的应用效果。方法选取2016年6月至2019年6月间陕西省铜川矿务局中心医院收治的84例行食管癌根治术且术后行放疗治疗的患者,采用随机数表法分为研究组和对照组,每组42例。对照组患者采用常规护理,研究组患者在此基础上采用追踪营养护理方案,比较两组患者放疗前后营养指标[白蛋白(Alb)、血红蛋白(Hb)和总蛋白(TP)]、生化指标[三酰甘油(TG)、总胆固醇(TC)和血磷(SP)]、人体测量学指标[肱三头肌皮褶厚度(TSF)、上臂肌肉周经(MAMC)和体质指数(BMI)]变化及放疗期间并发症情况。结果放疗后,研究组患者Alb、Hb和TP指标均高于对照组,差异均有统计学意义(均P<0.05)。放疗后,两组患者TG和TC比较,差异无统计学意义(P>0.05);研究组患者SP低于对照组,差异有统计学意义(P<0.05)。放疗后,研究组患者TSF、MAMC和BMI指标均高于对照组,差异均有统计学意义(均P<0.05)。研究组患者放疗期间并发症发生率为9.5%,低于对照组的28.6%,差异有统计学意义(P<0.05)。结论追踪营养护理方案能改善食管癌术后放疗患者的营养状况,降低并发症概率,值得推广。
Objective To explore the application of nutrition follow-up nursing in esophageal cancer patients receiving postoperative radiotherapy.Methods From June 2016 to June 2019,84 patients who underwent esophagectomy and received postoperative radiotherapy at Tongchuan Mining Bureau Central Hospital were selected and were randomly divided into a study group and a control group with 42 patients in each group.The study group was given nutrition follow-up nursing besides conventional nursing and the control group was on given conventional nursing.The changes in nutritional indexes including albumin(ALB),hemoglobin(HB),and total protein(TP),biochemical indexes including triglyceride(TG),total cholesterol(TC)and serum phosphorus(SP)and anthropometric indexes including triceps skinfold thickness(TSF),upper arm muscle circumference(MAMC)and body mass index(BMI)before and after radiotherapy and incidence of complications during radiotherapy were compared between the two groups.Results After radiotherapy,the levels of ALB,Hb and TP indexes were higher in the study group than in the control group(all P<0.05).After radiotherapy,there was no significant difference in TG and TC between the two groups(P>0.05);SP index was lower in the study group than in the control group(P<0.05).After radiotherapy,levels of TSF,MAMC and BMI were higher in the study group than in the control group(all P<0.05).The incidence of complications was 9.5%in the study group which was lower than 28.6%of the control group(P<0.05).Conclusion The nutrition follow-up nursing has,can improve the nutritional status and reduce the incidence of complications in esophageal cancer patients treated by postoperative radiotherapy.It is worthy of clinical popularization.
