摘要
目的探讨快速康复理念对食管癌全胸腹腔镜手术术后患者肺功能的影响。方法选取2017年1月至2019年6月间山东省立第三医院收治的70例食管癌患者,采用随机数表法分为研究组和对照组,每组35例。两组患者均采用全胸腹腔镜手术治疗,对照组患者采用常规护理,研究组患者采用快速康复理念护理,比较两组患者治疗后拔管、排气功能恢复、下床活动、住院时间、肺活量(VC)、最大通气量(MVV)、1秒钟呼气容积(FEV1)、临床并发症发生率和生活质量情况。结果研究组患者肺活量、最大通气量和1秒呼气容积等肺功能均优于对照组,差异均有统计学意义(均P<0.05)。研究组患者拔管时间、排气功能恢复时间、下床时间和住院时间等手术指标均短于对照组,差异均有统计学意义(均P<0.05)。研究组患者并发症发生率为5.7%,低于对照组的22.9%,差异有统计学意义(P<0.05)。治疗后,两组患者生活质量评分均高于治疗前,且研究组各评分均高于对照组,差异均有统计学意义(均P<0.05)。结论全胸腹腔镜手术治疗食管癌患者术后采用快速康复理念护理,可提高患者肺功能和生活质量,减少临床并发症,应推广应用。
Objective To investigate the effect of fast rehabilitation concept on lung function in patients with esophageal cancer after the total thoracoscopic and laparoscopic surgery.Methods Between January 2017 and June 2019,70 patients with esophageal cancer were selected at Shandong Provincial Third Hospital.They were divided into a study group and a control group with 35 patients in each group.Total thoracoscopic and laparoscopic surgery was performed in both groups.The study group was given fast rehabilitation concept-based nursing and the control group was given conventional nursing.Time to extubation,recovery of the function of the exhaust system and out-of-bed activity,length of hospital stay,vital capacity(VC),maximum ventilation volume(MVV),forced expiratory volume in one second(FEV1),clinical complications and quality of life were compared between the two groups after treatment.Results Lung function including VC,MVV and FEV1 was significantly better in the study group than in the control group(all P<0.05).Time to extubation,recovery of the function of the exhaust system and out-of-bed activity and length of hospital stay was shorter in the study group than in the control group(all P<0.05).The incidence of complications was 5.7%in the study group which was lower than 22.9%of the control group(P<0.05).After treatment,scores of quality of life increased in both groups compared with before the treatment with the study group higher than the control group(all P<0.05).Conclusion Fast rehabilitation concept-based nursing can effectively improve lung function and quality of life,and reduce the incidence of clinical complications in patients with esophageal cancer after the total thoracoscopic and laparoscopic surgery.It is worthy of clinical popularization.
