摘要
目的探讨体素内不相干运动(intravoxel incoherent motion,IVIM)预测乳腺肿块良恶性及乳腺癌免疫组化指标价值。方法选取乳腺肿块患者96例为研究对象,使用IVIM序列对其进行检测,比较分析良恶性病变及不同免疫组化指标表达患者快扩散系数(D*)、慢扩散系数(D)、灌注分数(f)及表观扩散系数(ADC)值,并绘制ROC曲线分析IVIM各参数诊断价值。结果恶性病变患者D=(0.93±0.19)×10-3 mm 2·s-1,显著低于良性者,f=(22.96±4.28)%,显著高于良性者,差异有统计学意义(P<0.05),2组D*值差异无统计学意义(P>0.05);乳腺癌的雌激素受体(estrogen receptor,ER)、孕激素受体(progesterone receptor,PR)不同表达下,IVIM各指标水平差异无统计学意义(P>0.05);人类表皮生长因子受体2(human epidermal growth factor receptor-2,HER-2)阳性者D=(0.91±0.20)×10-3 mm 2·s-1、D*=(32.55±20.08)×10-3 mm 2·s-1,均高于表达阴性者,差异有统计学意义(P<0.05),f值差异无统计学意义(P>0.05);细胞增殖抗原标记物Ki-67(antigen identifed by monoclonal antibody Ki-67,Ki-67)不同表达者D*、f值差异无统计学意义(P>0.05),Ki-67高表达者D=(0.78±0.12)×10-3 mm 2·s-1,显著低于低表达者,差异有统计学意义(P<0.05)。结论D、D*、f、ADC 4个指标中,D值诊断效能较高,可作为临床诊断乳腺肿块良恶性鉴别的独立指标。D值与Ki-67的表达具有密切联系。
Objective To investigate the value of intravoxel incoherent motion(IVIM)in predicting the benign and malignant breast masses and immunohistochemical indexes of breast cancer.Methods A total of 96 patients with breast masses were selected as the research objects.IVIM was used to detect the benign and malignant lesions to compare and analyze the benign and malignant breast masses and immunohistochemical index,which expressed the perfusion-related diffusion coefficient(D*),perfusion-independent diffusion coefficient(D),perfusion fraction(f)and standardized diffusion coefficient(ADC)values,and the ROC curve was used to analyze the diagnostic value of IVIM parameters.Results D=(0.93±0.19)×10-3 mm 2·s-1 in patients with malignant lesions,significantly lower than the benign ones,f=(22.96±4.28)%,significantly higher than the benign ones,the difference was significant(P<0.05),the difference in D*values between the two groups was not significant(P>0.05);under different expressions of estrogen receptor(ER)and progesterone receptor(PR)of breast cancer,the difference was not statistically significant in the levels of IVIM indicators(P>0.05);human epidermal growth factor receptor-2(HER-2)positive D=(0.91±0.20)×10-3 mm 2·s-1 and D*=(32.55±20.08)×10-3 mm 2·s-1,were significantly higher than negative ones,the difference was not statistically significant(P<0.05),there was no significant difference in f value,antigen identified by monoclonal antibody Ki-67(Ki-67)was not significantly different in D*and f values(P>0.05)for different expressions,D=(0.78±0.12)×10-3 mm 2·s-1 for Ki-67 high expressions were significantly lower than those with low expressions,the difference was not significant(P<0.05).Conclusion Among D,D*,f and ADC indicators,D value has high diagnostic efficiency,and it can be used as an independent indicator for clinical diagnosis of the benign and malignant breast masses.
