摘要
目的了解变态反应性皮肤病患者血清总IgE和特异性IgE(sIgE)的种类、分布与相互关系,为临床诊疗提供实验室依据。方法采用欧蒙过敏原检测系统对珠海市慢性病防治中心皮肤科就诊的变态反应性皮肤病患者进行血清总IgE和特异性sIgE检测,按年龄将患者分为婴幼儿、青少年、中青年和老年组,分析组间特异性变应原的分布;将总IgE数值分为正常组、2倍参考值组、3倍参考值组、大于3倍参考值组,分析总IgE和特异性IgE(sIgE)的关联性。结果985例患者中总IgE检测有反应性578例,sIgE检测有反应性716例。吸入性过敏原最多的是尘螨组合(屋尘螨/粉尘螨),其次是蟑螂,食入性过敏原最多的是海鱼组合1(鳕鱼/龙虾/扇贝),其次是淡水鱼组合1(鲑鱼/鲈鱼/鲤鱼);吸入性过敏原各年龄组中最多的为尘螨组合,食入性过敏原中,婴幼儿组最多是鸡蛋白,其余三组为海鱼组合。IgE浓度各浓度的占比不随总IgE浓度改变而变化,除总IgE的浓度大于3倍参考值时sIgE浓度6级(非常高的抗体滴度)数量明显增加。结论尘螨组合(屋尘螨/粉尘螨),蟑螂,海鱼组合1(鳕鱼/龙虾/扇贝),和淡水鱼组合1(鲑鱼/鲈鱼/鲤鱼)是变态反应性患者最常见的过敏原物质,当总IgE的浓度大于3倍参考值时sIgE 6级(非常高的抗体滴度)数量明显增加。血清特异性过敏原的检测对个体化治疗和健康教育具有重要作用。
Objective To investigate the kind,distribution and relationship of IgE and sIgE in patients with allergic skin disease,in order to provide laboratory basis for diagnosis and treatment.Methods EURO Allery Screen system was used to detect serum total IgE and allergen-specific IgE(sIgE)of patients with allergic skin disease in the hospital.According to the age,the patients were divided into infant group,teenager group,middle-young-aged group and older age group.The result of sIgE in serum between groups was compared.The result of total IgE was divided into normal group,double reference value group,triple reference value group and more than triple reference value group.The relevancy between groups was compared.Results Among 985 patients,578 were total IgE positive and 716 were sIgE positive.The most common inhaled allergens were dust mites(including dermatophagoides,pteronyssinus and dermatophagoides farina),followed by cockroach.The most common food allergens were sea fish(including cod,lobster and scallops),followed by freshwater fish(salmon,bass and carp).For inhaled allergens,the highest positive rate was dust mites among every age group.For food allergens,the highest positive rate was egg white in infant group,and sea fish in other groups.When the concentration of the total IgE was more than triple reference value,the number of sIgE level 6(very high antibody titer)increased significantly,and the rest was basically the same.Conclusion Dust mites,cockroach,sea fish and freshwater fish are the most common allergens in Zhuhai Area.The number of sIgE level 6(very high antibody titer)increases significantly when the concentration of the total IgE is more than Triple reference value.The detection of serum specific allergens plays an important role in personalized treatment and health education.
