摘要
目的探讨去甲肾上腺素联合肾上腺素对感染性休克患儿组织微循环障碍、肠内喂养的影响。方法选取2014年7月至2018年9月在该院治疗的1个月龄至18岁感染性休克患儿180例,按照随机数字表法分为对照组和观察组,各90例。对照组给予肾上腺素治疗,观察组在对照组基础上联合去甲肾上腺素治疗。比较两组患者治疗后7 d心率、舒张压、心脏指数、组织微循环情况、治疗预后、肠内喂养率及并发症发生率。结果观察组治疗后7 d心率、血乳酸水平低于对照组,舒张压、心脏指数、尿量高于对照组,差异有统计学意义(P<0.05);观察组治疗后7 d PVD、PPV水平均高于对照组,肠内喂养率、低血压、心动过速、心室颤动、心律失常发生率均低于对照组,差异有统计学意义(P<0.05)。结论去甲肾上腺素联合肾上腺素用于感染性休克患儿中有助于改善患者生命体征及组织微循环,能降低肠内喂养率及并发症发生率,值得临床推广应用。
Objective To investigate the effects of norepinephrine combined with epinephrine on tissue microcirculation disorder and enteral feeding in children with septic shock.Methods From July 2014 to September 2018,180 children aged from 1 month to 18 years old in a hospital with septic shock were selected and divided into control group and observation group according to the random number table method,with 90 cases in each group.The control group was treated with epinephrine,and the observation group was treated with norepinephrine on the basis of the control group.Seven days after of treatment,the heart rate,diastolic blood pressure,heart index,tissue microcirculation,treatment prognosis,enteral feeding rates and complication rates of the two groups were compared.Results Seven days after of treatment,the heart rate and blood lactate levels in the observation group were lower than those in the control group,and diastolic blood pressure,heart index and urine volume were higher than those in the control group,with statistically significant differences(P<0.05).Seven days after treatment,the levels of PVD and PPV in the observation group were higher than those in the control group,and the rates of enteral feeding,hypotension,tachycardia,ventricular fibrillation and arrhythmia were all lower than those in the control group,with statistically significant differences(P<0.05).Conclusion Norepinephrine combined with epinephrine in children with septic shock could improve vital signs and tissue microcirculation of patients,reduce the rate of enteral feeding and the incidence of complications,which is worthy of clinical promotion and application.
