摘要
目的观察针刺结合中医正骨手法治疗腰椎间盘突出症(LDH)的临床疗效。方法将82例LDH患者按照随机数字表法分为2组。对照组41例予中医正骨手法治疗,治疗组41例在对照组基础上配合针刺治疗。2组均2周为1个疗程,共治疗2个疗程。比较2组治疗前后日本骨科协会(JOA)下腰痛评分、疼痛视觉模拟评分(VAS)及血清白细胞介素6(IL-6)、IL-8、超敏C反应蛋白(hs-CRP)变化情况,统计比较2组临床疗效。结果2组治疗2周、治疗后的JOA下腰痛评分、疼痛VAS均与本组治疗前比较差异有统计学意义(P<0.05),JOA评分升高,VAS评分降低;治疗2周及治疗后2组组间同一时间点JOA下腰痛评分、VAS比较差异有统计学意义(P<0.05)。治疗后2组血清IL-6、IL-8、hs-CRP水平均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗组总有效率97.56%(40/41),对照组总有效率82.93%(34/41),治疗组总有效率高于对照组(P<0.05)。结论针刺配合中医正骨手法治疗LDH疗效确切,能够改善腰部功能,缓解疼痛,其作用机制可能与降低炎症因子水平有关。
Objective To observe the clinical efficacy of acupuncture combined with bone setting manipulation in the treatment of lumbar disc herniation(LDH).Methods 82 patients with LDH were divided into two groups according to the random number table method.41 patients in the control group were treated by bone setting manipulation,while 41 cases in the treatment group were treated by acupuncture on the basis of the control group.Two weeks was a course of treatment in both groups,with a total of 2 courses.The Japanese orthopaedic association(JOA)low back pain scores and VAS scores before and after treatment were analyzed,and clinical efficacy of the interleukin-6(IL-6),interleukin 8(IL-8),high-sensitivity C reactive protein(hs-CRP)were compared.Result After treatment 2 weeks and after treatment there were significant differences in JOA score and VAS score between the two groups at before treatment(P<0.05),the JOA scores increased,VAS score decreased;there were significant differences in JOA score and VAS score between the two groups after treatment 2 weeks and after treatment(P<0.05),and there was interaction between time and grouping factors(P<0.05).After treatment,the serum levels of IL-6,IL-8 and hs CRP in the two groups were lower than those before treatment(P<0.05),and the treatment group were lower than the control group(P<0.05).The total effective rate of the treatment group was 97.56%(40/41),and that of the control group was 82.93%(34/41).The total effective rate of the treatment group was higher than that of the control group(P<0.05).Conclusion Acupuncture combined with bone setting manipulation is effective in treating LDH,which can improve lumbar function and relieve pain,and its mechanism may be related to the reduction of inflammatory factors.
