摘要
目的将徒手盲插鼻空肠管技术用于重症胰腺炎护理中,对其可行性及安全性进行分析。方法选取2017年4月—2018年5月该院收治的70例重症胰腺炎患者分组研究,数字随机表法分为观察组(n=35)与对照组(n=35),对照组行常规留置胃管,观察组行徒手盲插鼻空肠管技术,对两组一次性置管成功率、置管并发症发生率、营养状况进行对比分析。结果观察组一次性置管成功率为94.29%,与对照组100.00%相比,差异无统计学意义(P>0.05);观察组并发症率为5.71%,与对照组25.71%相比,相对更低,差异有统计学意义(χ~2=5.285,P<0.05);观察组营养状况A级、B级、C级与对照组相比,差异无统计学意义(P>0.05)。结论徒手盲插鼻空肠管技术可获得与常规留置胃管类似的效果,在重症胰腺炎患者护理应用中具有可行性,一次性置管成功率较高,可减少误吸、肺部感染等并发症,具有较高安全性,还可满足患者营养需求,利于促进患者早日康复。
Objective To apply the technique of blind insertion of nasal jejunal tube in nursing care of severe pancreatitis,and analyze its feasibility and safety.Methods A group study of 70 patients with severe pancreatitis admitted from April 2017 to May 2018 in the hospital was selected.The digital random table method was divided into an observation group(n=35)and a control group(n=35).For gastric tube,the observation group performed the technique of blindly inserting nasal jejunal tube with free hand,and compared the success rate of one-time catheter placement,incidence of catheter complications,nutritional status between the two groups.Results The success rate of one-time catheter placement in the observation group was 94.29%,which was not statistically significant compared with 100.00%in the control group(P>0.05);the complication rate in the observation group was 5.71%,compared with 25.71%in the control group.The difference was statistically significant(χ~2=5.285,P<0.05).Compared with the control group,the nutritional status of the observation group A,B,and C was not statistically significant(P>0.05).Conclusion The technique of blind insertion of the nasal jejunal tube by hand can obtain the same effect as the conventional indwelling gastric tube,and it is feasible in the nursing application of patients with severe pancreatitis disease,with high safety,and which can also meet the nutritional needs of patients,which is conducive to the early recovery of patients.
