摘要
目的观察生脉注射液联合无创正压机械通气对急性心力衰竭患者的临床疗效。方法采用前瞻性研究方法,选取我院2017年3月—2019年3月住院治疗的患者60例,第一诊断均为急性心力衰竭。依据随机数字表法随机分为对照组和治疗组,每组30例。对照组患者入院后均给予常规治疗,同时加用无创正压机械通气治疗;治疗组在对照组治疗基础上联合应用生脉注射液50 mL日2次静脉泵入,2组均连用2 d。比较两组患者治疗后2 h、2 d的症状,体征,血气分析指标和血N端前脑钠肽(N-terminal pro brain natriuretic peptide,NT-ProBNP)的变化。结果治疗2 h结束后两组患者症状、体征较治疗前改善(P<0.05),治疗组改善更明显(P<0.05);治疗2 d后患者症状、体征、血气分析指标及血NT-ProBNP均较治疗前改善(P<0.05),治疗组改善更明显(P<0.05)。结论生脉注射液联合无创正压机械通气对急性心力衰竭疗效明显,可提升患者心脏功能,增加心排血量,改善血流动力学指标,提升组织及细胞灌注,迅速纠正低氧血症,快速缓解急性心力衰竭症状及体征,可提高急性心力衰竭患者临床抢救成功率。
Objective To observe the clinical effect of Shengmai Injection combined with noninvasive positive pressure ventilation(NPPV)on acute heart failure. Methods A prospective study was conducted to select 60 patients with acute heart failure who were treated in our department from March 2017 to March 2019. They were divided into control group and treatment group according to random number table,30 cases in each group. The patients in the control group were given routine treatment and non-invasive ventilatorassisted breathing therapy after admission;the patients in the treatment group were given Shengmai Injection twice a day on the basis of the treatment in the control group,and the patients in both groups were given Shengmai injection twice a day for two days. The changes of symptoms,signs,blood gas analysis indexes and NT-ProBNP were compared between the two groups at 2 hours and 2 days after treatment. Results After 2 hours of treatment,the symptoms and signs of the two groups were improved compared with those before treatment(P<0.05),and the improvement of the treatment group was more obvious(P<0.05);after 2 days of treatment,the symptoms,signs,blood gas analysis indicators and blood NT-ProBNP of the two groups were improved compared with those before treatment(P<0.05),and the improvement of the treatment group was more obvious(P<0.05). Conclusion Shengmai Injection combined with non-invasive positive pressure ventilation has obvious curative effect on acute heart failure. It can rapidly improve the clinical symptoms and signs of patients,correct hypoxemia and improve cardiac function,and increase the success rate of rescue.
