摘要
目的分析溃疡清创联合角膜穿透移植术治疗真菌性角膜炎的临床效果。方法选取2018年3月-2019年3月在永州职业技术学院附属医院接受诊治的真菌性角膜炎患者90例,按照完全随机法分为对照组、联合组,各45例。对照组患者给予角膜穿透移植术,联合组在对照组的基础上给予患者角膜溃疡清创术。检测术后视力情况,观察记录手术次数、创口愈合时间、畏光眼泪、角膜溃疡、前房积脓以及眼部异物感等临床症状消失时间。统计治疗效果及不良反应发生率。结果两组患者手术后,联合组视力<0.3、~0.8、≥1.0患者与对照组比较,无统计学差异;联合组视力~0.5患者低于对照组,差异具有统计学意义(P<0.05);联合组患者手术次数、创口愈合时间、畏光眼泪、角膜溃疡、前房积脓以及眼部异物感等临床症状消失时间低于对照组(P<0.05);联合组治疗总有效率高于对照组(P<0.05);联合组治疗并发症发生率与对照组比较,无统计学差异。结论溃疡清创联合角膜穿透移植术对真菌性角膜炎患者治疗效果显著,可提高术后视力情况,显著改善患者临床症状,值得推广。
OBJECTIVE To observe the clinical effect of ulcer debridement combined with penetrating keratoplasty on treatment of fungal keratitis.METHODS A total of 90 patients with fungal keratitis who were treated in Yongzhou Vocational Technical College Medical and Technical College from Mar 2018 to Mar 2019 were enrolled in the study and randomly divided into the control group and the combined group,with 45 cases in each group.The control group was given corneal penetrating transplantation,and the combined group was given corneal ulcer debridement on the basis of the treatment of the control group.The postoperative visual acuity was detected,and the number of times of operations,wound healing time,photophobia tears,corneal ulcer,anterior chamber empyema and ocular foreign body sensation were observed,and the curative effect and incidence of adverse reactions were statistically analyzed.RESULTS There was no significant difference in the proportion of the patients with the vision less than 0.3,0.8,and no less than 1.0 between the combined group and the control group after the surgery.The proportion of the patients with vision 0.5 was significantly lower in the combined group than in the control group(P<0.05).The number of times of surgery,wound healing time,and the time of disappearance of clinical symptoms such as photophobia tears,corneal ulcers,anterior chamber empyema and ocular foreign body sensation were significantly lower in the combined group than in the control group(P<0.05).The total effective rate of treatment of the combined group was significantly higher than that of the control group(P<0.05);there was no significant difference in the incidence of treatment-induced complications between the combined group and the control group.CONCLUSION Ulcer debridement combined with penetrating keratoplasty can achieve significantly effect on treatment of the patients with fungal keratitis,improve the postoperative visual acuity,and significantly improve the clinical symptoms of the patients,and it is worthy to be promoted.
