摘要
目的 研究甘精胰岛素与磷酸西格列汀二联疗法对老年2型糖尿病(T2DM)患者糖代谢及空腹C肽和餐后2h C肽的影响.方法 选取从2017年2月~2019年2月收治的82例老年T2DM患者进行研究,将其以随机数表法等分成二联组及对照组.对照组实施甘精胰岛素注射治疗,二联组则采用甘精胰岛素与磷酸西格列汀二联疗法治疗.两组均进行12周的治疗,比较治疗前后糖代谢及空腹C肽和餐后2 h C肽水平,治疗前后视黄醇结合蛋白4(RBP4)、Nesfatin-1水平,不良反应发生情况.结果 治疗后二联组空腹血糖(FBG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)水平均显著低于对照组(P<0.05).治疗后二联组空腹C肽、餐后2 h C肽均显著高于对照组(P<0.05).治疗后二联组RBP4、Nesfatin-1水平均显著低于对照组(P<0.05).二联组与对照组在腹痛、恶心呕吐、便秘发生率方面对比,差异均不明显(P>0.05).结论 甘精胰岛素与磷酸西格列汀二联疗法可显著改善老年T2DM患者糖代谢及空腹C肽和餐后2 h C肽水平,同时可改善RBP4、Nesfatin-1水平,安全性较佳.
Objective To study the effect of insulin glargine combined with Phosphoric acid Siglitatin on glucose metabolism,fasting c-peptide and 2 h post-meal c-peptide in elderly patients with type 2 diabetes mellitus(T2DM).Methods A total of 82 elderly T2DM patients admitted to our hospital from February 2017 to February 2019 were selected for the study.The control group was treated with insulin glargine injection,while the combined group was treated with insulin glargine combined with Phosphoric acid Siglitatin.Both groups were treated for 12 weeks.Glucose metabolism,fasting c-peptide and 2 h C-peptide levels before and after treatment,retinol-binding protein 4(RBP4)and Nesfatin-1 levels before and after treatment and adverse reactions occurred rate were compared.Results The fasting blood glucose(FBG),2 h postprandial blood glucose(2hPG)and glycosylated hemoglobin(HbA1c)levels in the combined group were significantly lower than those in the control group(P<0.05).After treatment,fasting c-peptide and 2 h C-peptide were significantly higher in the combined group than those in the control group(P<0.05).After treatment,the levels of RBP4 and Nesfatin-1 in the combined group were significantly lower than those in the control group(P<0.05).Compared with the control group,the combined group of incidence of abdominal pain,nausea and vomiting and constipation was not significantly different(P>0.05).Conclusion Insulin glargine combined with Phosphoric acid Siglitatin can significantly improve glucose metabolism,fasting c-peptide and 2h post-meal c-peptide levels in elderly T2DM patients,and improve RBP4 and nesatin-1 levels.The safety is better and the clinical application value is higher.
