摘要
目的探讨吊顶灯辅助23G玻切治疗开放性Ⅱ期重度眼外伤伴束状视网膜脱离的最佳手术时间、临床疗效及并发症情况。方法回顾性分析2016年1月~2019年1月在我院眼科中心住院的40例(40眼)开放性Ⅱ期重度眼外伤伴束状视网膜脱离患者的临床资料,行吊顶灯辅助23G玻璃体切割手术,根据患者行Ⅱ期玻切手术时间点不同,将研究资料分为2组,观察组时间为5~7d,平均5.5d;对照组为10~15d,平均11.5d。随访6个月,分析两组术前和术后最佳矫正视力及视网膜解剖复位的变化。结果两组术后6个月最佳矫正视力均较术前提高,差异有显著性(P<0.05);两组术后6个月最佳矫正视力对比,差异无显著性(P>0.05);两组术后6个月视网膜解剖复位率,差异有显著性(P<0.05)。两组术中均无出血,术后无切口渗漏、眼内炎等并发症。结论吊顶灯辅助23G玻切治疗开放性Ⅱ期重度眼外伤伴束状视网膜脱离具有微创、缩短手术时间、术后效果佳等优点,是一种安全有效的手术方法。
Objective To explore the best operation time,clinical effect and complications of ceiling lamp assisted 23 G vitrectomy in the treatment of open stageⅡsevere ocular trauma with fascicular retinal detachment.Methods A retrospective analysis of 40 patients(40 eyes)with open stage II severe eye injury and fascicular retinal detachment was carried out who were hospitalized in the Ophthalmic Center of our hospital from January 2016 to January 2019.The study data were divided into two groups according to the different time points of stage II vitrectomy,the experimental group lasted 5~7 days,with an average of 5.5 days;The control group was 10~15 days,with an average of 11.5 days.After 6-months of follow-up,the changes of the best corrected visual acuity and the anatomic reduction of retina in the two groups were analyzed.Results The best corrected visual acuity of the two groups at 6 months after operation was higher than before the operation,the difference was statistically significant(P<0.05);But there was no significant difference for the best corrected visual acuity at 6 months between the two groups after operation(P>0.05).The anatomical reduction rate of the retina at 6 months after operation in the two groups was significantly different(P<0.05).There were no bleeding,incision leakage,endophthalmitis and other complications in both groups.Conclusion Ceiling lamp assisted 23 G vitrectomy is safe and effective for open stageⅡsevere ocular trauma with bundle retinal detachment,which has the advantages of minimally-invasive wound,shortened operation time,and good postoperative effect.
