摘要
目的 分析体外受精前禁欲时间小于24小时对治疗结局的影响.方法 回顾性收集2018年5至10月在西北妇女儿童医院生殖中心进行辅助生殖技术(ART)治疗夫妇的数据资料,根据取卵前最后一次排精至取卵日取精的间隔时间,将患者分为短时禁欲组(<24小时)和常规禁欲组(2~7天),比较两组间的受精率、正常受精率、可用胚胎率、囊胚形成率、人绒毛膜促性腺激素(hCG)阳性率、临床妊娠率、自然流产率的差异.结果 短时禁欲组的可用胚胎率显著高于常规禁欲组[(0.71±0.25)%vs.(0.67±0.27)%,t=2.329,P<0.05],特别是在体外受精(IVF)患者中两组差异也有统计学意义[(0.68±0.24)%vs.(0.64±0.26)%,t=2.288,P<0.05];在单精子卵胞浆内注射受精(ICSI)患者中,短时禁欲组的囊胚形成率显著高于常规禁欲组[(0.61±0.32)%vs.(0.51±0.29)%,t=2.270,P<0.05];余受精率、正常受精率、hCG阳性率、临床妊娠率、早期流产率等差异均无统计学意义(均P>0.05).结论 取卵前禁欲时间小于24小时并未影响ART助孕临床妊娠结局,甚至能够提高IVF后的可用胚胎率和ICSI受精后的囊胚形成率.
Objective To investigate effect of abstinence time less than 24 hours before in vitro fertilization(IVF)on treatment outcomes of assisted reproduction.Methods The data of infertile couples who received assisted reproductive technique(ART)treatment in our center from May to October,2018 were retrospectively analyzed.According to abstinence interval from last ejaculation before oocyte retrieval to semen collection on the ovum pick-up day,the patients were divided into two groups:short abstinence group(<24 hours)(n=370)and routine abstinence group(2~7 days)(n=2434),the fertility rate,normal fertility rate,available embryo rate,blastocyst formation rate,hCG positive rate,clinical pregnancy rate,spontaneous abortion rate of the patients were compered between the two groups.Results The available embryo rate of the patients in the short abstinence group was significant higher than that in the routine abstinence group(0.71±0.25 vs 0.67±0.27,t=2.329,P<0.05),especially in these patients who received IVF there was a significant difference between the two groups(0.68±0.24 vs 0.64±0.26,t=2.288,P<0.05).In those patients who received intracytoplasmic sperm injection(ICSI),the blastocyst formation rate in the short abstinence group was higher than that in the routine abstinence group(0.61±0.32 vs 0.51±0.29,t=2.270,P<0.05).While there were no significant differences in the fertility rate,normal fertility rate,hCG positive rate,clinical pregnancy rate,spontaneous abortion rate between the two groups(all P>0.05).Conclusion Abstinence time less than 24 hours before IVF do not affect clinical pregnancy outcomes of ART proconception,even more,shorter abstinence time may improve available embryo rate after IVF and blastocyst formation rate after ICSI.
