延续性Kegel运动训练联合神经肌肉电刺激对产后盆底肌康复的临床价值 被引量:43

Clinical value of continuous Kegel exercise training and neuromuscular electrical stimulation on postnatal pelvic floor muscle rehabilitation

摘要 目的探讨延续性Kegel运动训练和神经肌肉电刺激对产后盆底肌康复的联合应用效果。方法前瞻性选择2019年1~8月于同济大学附属第一妇婴保健院分娩的产妇共176例,采用随机数字表法将患者分为研究组和对照组,每组各88例。研究组和对照组产妇均在分娩后第7天开始进行延续性Kegel运动训练,研究组产妇在此基础上于产后8周起采用盆底肌肉电刺激进行康复治疗。观察并分析两组产妇的盆底肌力改善情况和尿失禁发生情况,计算总有效率。结果研究组产妇的Ⅰ、Ⅱ型肌纤维的显效比率(39.77%,35.23%)均明显高于对照组(30.68%,26.14%),差异均有显著性(P<0.05),总有效率(94.32%)也明显高于对照组(82.95%),差异有显著性(P<0.05)。研究组产妇不漏尿比率(57.95%)明显高于对照组(42.05%),中度漏尿比率(9.09%)明显低于对照组(18.18%),差异均有显著性(P<0.05),两组产妇轻度漏尿的发生率相比(32.96%vs.39.77%),差异无显著性(P>0.05)。结论延续性Kegel运动训练联合神经肌肉电刺激在产后盆底肌康复相较单一延续性Kegel运动训练具有更好的效果,值得临床医师参考使用。 Objective To explore the combined effect of continuous Kegel exercise training and neuromuscular electrical stimulation on postpartum pelvic floor muscle rehabilitation.Methods A total of 176 women who delivered in Tongji University Affiliated to the First Maternal and Infant Health Hospital from January to August 2019 were selected prospectively and divided into a study group and a control group by random number method,88 cases in each group.Both in the study group and the control group,Kegel exercise training started on the 7th day after delivery.On this basis,the study group used pelvic floor muscle stimulation for rehabilitation treatment from 8 weeks after delivery.The pelvic floor muscle strength improvement and urinary incontinence were observed and analyzed in both groups,the total effective rate was calculated.Results The significant effective rate of type I and type II muscle fibers in the study group(39.77%,35.23%)was significantly higher than that in the control group(30.68%,26.14%),the difference was significant(P<0.05).The total effective rate(94.32%)was also significantly higher than that in the control group(82.95%),the difference was significant(P<0.05).The rate of no leakage of urine(57.95%)in the study group was significantly higher than that in the control group(42.05%),and the rate of moderate leakage of urine(9.09%)was significantly lower than that in the control group(18.18%),the difference was significant(P<0.05).There was no significant difference between the two groups(32.96%vs.39.77%).Conclusion Continuous Kegel exercise training combined with neuromuscular electrical stimulation has better effects on postnatal pelvic floor muscle rehabilitation than continuous Kegel exercise training alone,and which is worthy of clinical application.
