摘要
总结何若苹治疗前列腺癌的经验。认为前列腺癌的病因主要为先天不足和饮食不节,病机为肝肾阴亏,正气失司,湿热盘踞,或饮食不节,损伤脾胃,久酿成湿,湿热下注。基于对前列腺癌"本虚标实"的病机认识,治疗遵循"不断扶正,适时攻邪,随证治之"的原则。"扶正"即扶正培本;"攻邪"涵盖了清热解毒、利水渗湿、化瘀通络、软坚散结、燥湿健脾等治法,对尿储留患者主要运用益肾软坚和清热利湿两种祛邪方法;"随症治之"即对术后尿失禁、化疗后肝功能异常、急性放射综合征、前列腺特异性抗原升高等情况进行随证处理。
This paper summarized HE Ruoping′s experience in treating prostate cancer.It is believed that the etiology is mainly related to inadequate natural endowment and improper diet,and the pathogenesis is yin deficiency of liver and kidney,dysfunction of vital qi,accululation of dampness and heat,improper diet,which damages the spleen and stomach.Or as time goes by,those becomes dampness,which diffuses downward with heat.Based on the understanding of pathogenesis of deficiency in origin and excess in superficiality of prostate cancer,the principle of"constantly strengthening vital qi and consolidating body resistance,timely attacking pathogens and treating them according to syndrome"was followed.Strengthening vital qi means strengthening vital qi and consolidating body resistance.Attacking pathogens timely means the treatment of clearing heat and removing toxicity,diuresis and diffusing dampness,dispersing blood stasis and removing obstruction in collateral,softening and dissipate stasis,and invigorating spleen by dryness and dampness.Two main methods of eliminating pathogenic factors are used for the patients with urinary retention i.e.tonifying the kidney to soften the stiffness,and clearing heat and promoting diuresis.Treating disease according to the syndromes includes treating post-operative urinary incontinence,liver dysfunction after che-motherapy,acute radiation syndrome and elevated prostate specific antigen,depending on the circumstance.
