摘要
目的探究川崎病(KD)患儿多种血液学指标水平与冠状动脉病变(CAL)的相关性。方法分析2017年6月-2018年12月间东莞市第三人民医院收治的42例KD患儿,根据是否发生CAL分为CAL组(n=14)和NCAL组(n=28),比较两组患儿一般临床资料及相关血液学指标。应用多因素Logistic回归模型分析KD患儿发生CAL的危险因素。结果两组患儿的性别比例、体重、完全性川崎病和不完全性川崎病比例的差异无统计学意义(P>0.05)。CAL组患儿平均年龄低于NCAL组,发热时间长于NCAL组,差异有统计学意义(P<0.05)。两组患儿白细胞、C反应蛋白和血沉水平差异无统计学意义(P>0.05),CAL组患儿的中性粒细胞百分比、血小板计数(PLT)、D-二聚体和N末端脑钠肽前体(NT-proBNP)水平高于NCAL组,差异有统计学意义(P<0.05)。多因素Logistic回归模型显示高中性粒细胞百分比(OR=1.409,P=0.032)、高PLT(OR=1.106,P=0.047)和高NT-proBNP(OR=4.021,P=0.014)是KD患儿发生CAL的危险因素。结论高NT-proBNP水平、高PLT和高中性粒细胞百分比是KD患儿发生CAL的危险因素。
Objective To explore the correlation between coronary artery lesions(CAL) and various blood parameters in children with Kawasaki disease(KD).Methods The clinical data of 42 children with KD treated in our hospital from June2017 to December 2018 were analyzed.They were divided into CAL group(14 cases) and NCAL group(28 cases)according to the occurrence of CAL.The general clinical data and related hematological indicators of the two groups were compared.Multivariate logistic regression model was used to analyze the risk factors of CAL in children with KD.Results There was no significant difference in sex ratio,body weight,cKD and iKD proportion between the two groups(P>0.05).The average age of children in CAL group was lower than that in NCAL group,and the fever time was longer than that in NCAL group(P<0.05).There was no significant difference in WBC,CRP and ESR levels between the two groups(P>0.05).The percentage of neutrophils,PLT,D-dimer and NT-proBNP levels in CAL group were higher than those in NCAL group(P<0.05).Multivariate logistic regression model showed that high percentage of neutrophils(OR=1.409,P=0.032),high PLT(OR=1.106,P=0.047) and high NT-proBNP(OR=4.021,P=0.014) were risk factors for CAL in children with KD.Conclusion High NT-proBNP,high PLT and high percentage of neutrophils were risk factors for CAL in children with KD.
