摘要
目的:探究替牙期单侧完全性唇腭裂(UCLP)患者上气道结构与舌骨位置的特征。方法:纳入30例7~12岁替牙期UCLP术后患儿(UCLP组),骨性Ⅰ类与骨性Ⅲ类非唇腭裂儿童各30例(骨性Ⅰ类组与骨性Ⅲ类组)。Winceph7.0软件进行头影测量,数据用单因素方差分析进行统计学分析。结果:与骨性Ⅰ类组相比UCLP组PNS-S、PNS-p1、ph1-ph2减小,PNS-Aa、ve1-ve2、p1-p2、舌骨测量指标增大;与骨性Ⅲ类相比,UCLP组PNS-S、PNS-p1、Sp1-Sp2、H-FH减小,PNS-Aa、ve1-ve2、p1-p2增大,以上结果均有统计学差异(P<0.05)。结论:与非唇腭裂替牙期儿童相比,替牙期UCLP术后儿童鼻咽部骨高度与舌咽部气道最短距减少,腭咽部气道宽度增加;舌骨位置偏后下。
Objective: To investigate the characteristics of upper airway structure and hyoid position in patients with unilateral cleft lip and palate(UCLP) during mixed dentition. Methods: 30 postoperative children aged 7-12 years with UCLP during mixed dentition were included as UCLP group. 30 children of skeletal Class Ⅰ and skeletal Class Ⅲ were included as skeletal Class Ⅰ group and skeletal Class Ⅲ group. Cephalometry with WinCeph 7.0 software was used to measure the airway morphology and hyoid position, data were statistically analyzed by one-way ANOVA. Results: Compared with the skeletal Class, PNS-S, PNS-p1, ph1-ph2 in UCLP group were decreased, while Pns-Aa, ve1-ve2, p1-p2 and hyoid measurements were increased. Compared with skeletal type III patients, Pns-s, Pns-p1, Sp1-Sp2 and H-FH were decreased, Pns-Aa, ve1-ve2, p1-p2 were increased in UCLP group. All the above results have statistical significance(P<0.05). Conclusion: Compared with non-cleft lip and palate mixed dentition children, the height of nasopharyngeal bone and the shortest distance of glossopharyngeal airway in children after UCLP repair in mixed dentition period in Urumqi area may decrease, but the upper airway of palatopharynx may increased. The hyoid bone position may be more backward and lower.
