摘要
目的探讨中医护理对胃癌术后胃瘫患者症状、临床疗效和生活质量的影响。方法选取2017年1月至2018年6月胃癌术后并发胃瘫综合征患者96例为研究对象,随机分为对照组48例和试验组48例。对照组患者采用常规治疗与护理,试验组患者在常规治疗和护理基础上应用2周穴位艾灸、外敷中药和中药灌肠中医护理方法,并评价两组患者胃瘫症状与临床疗效及生活质量。结果采用中医护理的胃癌术后胃瘫患者腹胀、腹痛、恶心呕吐和饮食情况显著低于对照组(P<0.01或P<0.05),疗效高于对照组(P<0.05);生活质量总体健康状况、各功能领域、症状领域(疲乏、恶心呕吐、食欲下降)评分低于对照组(P<0.01或P<0.05),但症状领域的疼痛、呼吸困难、失眠、腹泻和经济困难评分高于对照组(P>0.05)。结论穴位艾灸、外敷中药和中药灌肠的中医护理方法疗效显著、操作简单,可改变胃癌术后胃瘫患者临床症状,提高患者生活质量,值得临床推广应用。
Objective To investigate the effect of Traditional Chinese Medicine nursing on the symptoms,clinical efficacy and quality of life(QOL)of patients with gastric paralysis after gastric cancer surgery.Methods From January 2017 to June 2018,96 patients with gastroparesis syndrome after gastric cancer surgery were randomly divided into control group(48 cases)and experimental group(48 cases).The patients in the control group received routine treatment and nursing,while the patients in the experimental group received two weeks of point moxibustion,external application of Traditional Chinese Medicine and Traditional Chinese Medicine enema on the basis of routine treatment and nursing,to evaluate the symptoms,clinical efficacy and quality of life.Results The abdominal distension,abdominal pain,nausea,vomiting and diet of gastroparesis patients after gastric cancer operation nursed by Traditional Chinese Medicine were significantly lower than those of the control group(P<0.01 or P<0.05),and the curative effect was significantly higher than that of the control group(P<0.05).The overall health status,functional areas and symptom areas(fatigue,nausea and vomiting,loss of appetite)of QOL were significantly lower than those of the control group(P<0.01 or P<0.05).However,the score of symptom areas(pain,dyspnea,insomnia,constipation,diarrhea and economic difficulties)was higher than that of the control group(P>0.05).Conclusion Acupoint moxibustion,external application of Traditional Chinese Medicine and enema of Traditional Chinese Medicine have good curative effect,quick effect and simple operation,which can change the clinical symptoms of patients with gastroparesis after gastric cancer operation and improve the quality of life of patients.It is worthy of wide clinical promotion.
