摘要
目的探究心脏增大诊断应用CT平扫、X线及心电图价值差异。方法对自贡市中医医院和第五人民医院2016年7月至2019年7月期间收治98例疑似心脏增大各种心脏病患者相关影像学资料予以回顾性分析,患者均接受CT平扫、X线、心电图以及超声心动图检查。以超声心动图检查作为金标准判断CT平扫、X线、心电图对心脏增大诊断价值。结果以超声检查心脏检出情况作为金标准,CT平扫、X线及心电图对心脏增大检出符合率分别为94.34%、85.55%、70.75%,不同检查方式对心脏增大检出符合率比较具有统计学意义(χ~2=22.117,P<0.05);CT平扫对心脏增大诊断灵敏度与准确度明显优于X线与心电图诊断(P<0.05),X线诊断灵敏度与准确度明显优于心电图诊断(P<0.05),三种方式对于心脏增大诊断特异度,差异没有统计学意义(P>0.05)。结论 CT平扫、X线及心电图可为心脏增大诊断提供影像学参考,其中尤以CT平扫诊断价值最高。
Objective To explore differences in diagnostic value of CT plain scan,X-ray and electrocardiogram (ECG) for cardiac enlargement.Methods The relevant imaging data of 98 patients with various heart diseases and suspicion of cardiac enlargement who were admitted to Zigong Hospital of Traditional Chinese Medicine and Zigong Fifth People’s Hospital between July 2016 and July 2019 were analyzed retrospectively.All patients underwent CT plain scan,X-ray,ECG and echocardiography.Taking echocardiography as golden standard,diagnostic value of CT plain scan,X-ray and ECG for cardiac enlargement was determined.Results Taking cardiac detection conditions by ultrasound examination as golden standard,detection coincidence rates of CT plain scan,X-ray and ECG for cardiac enlargement were 94.34%,85.55% and 70.75%,respectively.There was statistical significance in detection coincidence rate of cardiac enlargement among all methods (χ~2=22.117,P<0.05).The sensitivity and accuracy of CT plain scan for diagnosis of cardiac enlargement were significantly better than those of X-ray and ECG (P<0.05).The sensitivity and accuracy of X-ray diagnosis were significantly better than those of ECG diagnosis (P<0.05).There was no significant difference in diagnostic specificity of cardiac enlargement among the three methods (P>0.05).Conclusion CT plain scan,X-ray and ECG can provide imaging reference for diagnosis of cardiac enlargement.Especially,diagnostic value of CT plain scan is the highest.
