摘要
目的研究治疗低钾血症补钾量的函数关系。方法参照纳入标准和排除标准入选35例。轻、中和重度低钾血症患者当日补钾所需氯化钾总量分别为4-6 g,6-8 g和8-12 g。测24 h尿钾和治疗前后血清钾浓度,计算出Δ细胞外液钾量、钾的治疗量(补钾总量-24 h尿钾-24 h尿钾/9)和Δ细胞内钾量(钾的治疗量-Δ细胞外液钾量),钾量的计算单位是mmol。设血清Δ细胞外液钾为自变量X,钾的治疗量为因变量Y。SPSS25.0解析正态分布,进行线性相关性、标准化残差和多种模型拟合效果比较分析。结果自变量和因变量呈正态分布、散点图分析和标准化残差分析显示显著的同向趋势,拟合回归比较显示线性相关最佳,治疗量与提升细胞外液新增量的函数关系式为Y=0.485+4.948X。结论Δ细胞外液钾与钾的治疗量存在线性相关,其线性方程式有助于临床细胞外液钾治疗量的评估。
Objective To study the functional relationship of potassium dosage in the treatment of hypokalemia.Methods A total of 35 cases were selected according to the inclusion and exclusion criteria.The total amount of potassium chloride for patients with light,moderate or severe hypokalemia was 4-6 g,6-8 g and 8-12 g,respectively.By measuring serum potassium level before and after treatment and 24 h urine potassium level,the inrecment of extracellular liquid potassium,the dosage of potassium supplement(=24 h total potassium supplement-24 h urine potassium level-one-ninth of 24 h urine potassium level)and intracellularly transferred potassium(=the dosage of potassium-the inrecment of extracellular liquid potassium)were calculated.The inrecment of extracellular liquid potassium was set to the independent variable X,and the dosage of potassium was the dependent variable Y.Normal distribution test,linear correlation,standardized residuals and multiple fitting were analyzed by SPSS25.Results The results showed that the independent variables and the dependent variables were normal distribution,the scatter plot analysis and the standardized residual analysis demonstrated significant trend in the same direction,and the fittings regression comparison concluded that variables were linearly dependent,the function equation between X and Y was Y=0.485+4.948 X.Conclusion There is a linear correlation between Y and X,and the linear equation is helpful for the clinical evaluation of the dosage of potassium supplement.
