摘要
目的:探讨七氟烷吸入麻醉与异丙酚静脉麻醉对腹腔镜手术患儿血流动力学和苏醒躁动的影响。方法:选取85例2017年5月-2019年2月于笔者所在医院进行腹腔镜手术的患儿作为研究对象,随机分为对照组42例与试验组43例。对照组行异丙酚静脉麻醉,试验组行七氟烷吸入麻醉,比较两组患儿麻醉、苏醒情况和血流动力学变化情况。结果:试验组麻醉起效时间、睁眼时间、清醒时间、定向力恢复时间早于对照组,PAED评分低于对照组(P<0.05)。两组T2、T3时刻MAP、EtCO2与T1时刻比较均有明显升高(P<0.05),试验组T2时刻HR与T1时刻比较均有明显升高(P<0.05),T3时刻HR与T1时刻比较差异无统计学意义(P>0.05),对照组T2、T3时刻HR与T1时刻比较均有明显升高(P<0.05),试验组HR、MAP波动比对照组小,SpO2、EtCO2波动与对照组比较差异无统计学意义(P>0.05)。结论:七氟烷吸入麻醉应用于腹腔镜手术患儿麻醉和苏醒质量好,对血流动力学影响小,能够降低手术应激反应和苏醒躁动程度,值得推广。
Objective:To investigate the effects of Sevoflurane inhalation anesthesia and Propofol intravenous anesthesia on hemodynamics and restlessness in children undergoing laparoscopic surgery.Method:A total of 85 children who underwent laparoscopic surgery in our hospital from May 2017 to February 2019 were selected as the study objects.According to the single blind method and random number table method,they were divided into the control group(42 cases)and the experimental group(43 cases).The control group received Propofol intravenous anesthesia,the experimental group received Sevoflurane inhalation anesthesia.Anesthesia,recovery and hemodynamic changes were compared between the two groups.Result:The onset time,opening time,awake time,recovery time of orientation force of the experimental group were earlier than those in the control group,and PAED score of the experimental group was lower than that in the control group(P<0.05).MAP and EtCO2 at T2 and T3 were significantly higher than those at T1(P<0.05),HR at T2 was significantly higher than that at T1(P<0.05),but there was no significant difference between HR at T3 and T1(P>0.05),HR at T2 and T3 in the control group was significantly higher than that at T1(P<0.05),HR and MAP fluctuations in the experimental group were smaller than those in the control group,and there was no significant difference in SpO2 and EtCO2 between the two groups(P>0.05).Conclusion:Sevoflurane inhalation anesthesia is of good quality in anesthesia and recovery of children undergoing laparoscopic surgery,with little stress response to surgery and little impact on hemodynamics.
