摘要
目的:总结林兰教授治疗糖尿病脑血管病变经验,通过糖尿病脑梗死临床病例回顾研究加以验证。方法:回顾性分析糖尿病脑梗死病例256例,与无大血管合并症的糖尿病对照逐步回归筛选各危险因素与糖尿病脑梗死发生的关系,并对糖尿病脑梗死中医证候分布和中医症证诊断进行分析。结果:糖尿病长期不良控制、胰岛素抵抗、肥胖、血液的"浓、黏、聚、滞"是糖尿病脑梗死的易患因素。糖尿病脑梗死气阴两虚型为40.23%;62.11%兼瘀血证;43.36%兼痰浊证。结论:气阴两虚为基本证型,瘀血、痰浊贯穿糖尿病脑梗死整个病程的始终。林兰教授这一学术思想通过糖尿病脑梗死临床病例研究得以证实。
Objective:To summarize professor LIN Lan’s experience in the treatment of diabetic cerebrovascular diseases,and to verify it by reviewing the clinical cases of diabetic cerebral infarction.Methods:A total of 256 cases of diabetic cerebral infarction were recruited,and the relationship between risk factors and the occurrence of diabetic cerebral infarction was analyzed by stepwise regression screening in comparison with the control group of non-diabetic macrovascular complications.Results:Long-term poor control of diabetes,insulin resistance,obesity,blood‘concentration,adhesion,aggregation,stagnation’is the risk factors of diabetic cerebral infarction.The deficiency of qi and yin syndrome was accounting for 40.23%;blood stasis syndrome was 62.11%;phlegm turbidity syndrome was 43.36%.Conclusion:The deficiency of qi and yin was the basic syndrome type,with the syndrome of blood stasis and phlegm turbidity throughout the whole course of diabetic cerebral infarction.Professor LIN Lan’s academic idea was confirmed by the clinical case study of diabetic cerebral infarction.
