摘要
目的对比超声TI-RADS分类与FNAB对甲状腺结节良恶性鉴别诊断中的应用价值。方法回顾性分析1034个甲状腺结节的常规超声检查TI-RADS分类及FNAB资料,根据术后病理结果进行比较。结果1034个结节中FNAB和TI-RADS分类结果分别与病理结果对比,差异有统计学意义(P<0.001),TI-RADS诊断的灵敏度为60.94%、特异度为67.08%、阳性预测值为90.94%、阴性预测值为24.05%、准确率为61.90%;FNAB诊断的灵敏度为86.48%、特异度为67.70%、阳性预测值为93.56%、阴性预测值为48.02%、准确率为83.56%;两者联合诊断的灵敏度为93.81%、特异度为44.72%、阳性预测值为90.80%、阴性预测值为59.09%、准确率为86.75%。结论TI-RADS与FNAB联合对甲状腺结节良恶性具有较高的诊断价值。
Objective To compare the value of ultrasound TI-RADS classification and FNAB in the differential diagnosis of benign and malignant thyroid nodules.Methods The Kwak version of TI-RADS classification and FNAB data of 1034 thyroid nodules were retrospectively analyzed,and compared based on postoperative pathological results.Results The classification results of FNAB and TI-RADS in 1034 nodules were compared with pathological results,and the differences were statistically significant(P<0.001).The sensitivity of TI-RADS diagnosis was 60.94%,the specificity was 67.08%,the positive predictive value was90.94%,the negative predictive value was 24.05%,and the accuracy rate was61.90%.The FNAB diagnostic sensitivity was 86.48%,the specificity was 67.70%,the positive predictive value was 93.56%,the negative predictive value was 48.02%,and the accuracy rate was 83.56%.The combined diagnosis had a sensitivity of93.81%,a specificity of 44.72%,a positive predictive value of 90.80%,a negative predictive value of59.09%,the accuracy rate was 86.75%.Conclusion The combination of TI-RADS and FNAB has high diagnostic value for benign and malignant thyroid nodules.
