摘要
目的分析常规超声结合超声造影在鉴别良恶性甲状腺结节(TNS)中的效果。方法回顾性分析2018年3月—2022年4月收治的病理检查证实为恶性TNS 88例(共106枚结节)及同一时期证实为良性TNS 92例(共112枚结节)的临床及超声资料。对比恶性与良性TNS常规超声及超声造影图像特征,并分析常规超声结合超声造影检查对良恶性TNS的诊断效果。结果恶性与良性TNS在结节内部成分、实性部分形态、实性回声、微钙化、血流信号、造影增强程度、造影均匀度、增强环、边界方面比较差异有统计学意义(P<0.01)。常规超声结合超声造影对良恶性TNS的诊断敏感度、特异度、准确度、阳性预测值和阴性预测值分别是93.52%、95.45%、94.50%、95.28%、93.75%,均高于常规超声检查(P<0.05)。结论常规超声结合超声造影在鉴别良恶性TNS中具有一定的价值,在TNS早期诊断和治疗方面具有重要意义。
Objective To analyze the effect of conventional ultrasound combined with contrast-enhanced ultrasound(CEUS)in differentiating benign and malignant thyroid nodules(TNS).Methods Retrospective analysis was performed on the clinical and ultrasonic data of 88 cases(106 nodules in total)of malignant TNS confirmed by pathology and 92 cases(112 nodules in total)of benign TNS confirmed by pathology from March 2018 to April 2022.The image characteristics of conventional ultrasound and CEUS were compared between benign and malignant TNS,and the diagnostic effect of conventional ultrasound combined with CEUS on benign and malignant TNS was analyzed.Results There were statistically significant differences between malignant and benign TNS in terms of internal components,solid part morphology,solid echo,microcalcification,blood flow signal,contrast enhancement degree,contrast uniformity,enhancement ring and boundary(P<0.01).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of conventional ultrasound combined with CEUS in the diagnosis of benign and malignant TNS were 93.52%,95.45%,94.50%,95.28%and 93.75%,respectively,which were higher than those of conventional ultrasound(P<0.05).Conclusion Conventional ultrasound combined with CEUS has a certain value in differentiating benign and malignant TNS,and is of great significance in early diagnosis and treatment of TNS.
