摘要
目的了解新疆阿克苏地区健康人群麻疹抗体水平,为制定麻疹免疫策略提供科学依据。方法采用分层随机抽样方法抽取阿克苏地区5个县健康人群1 200人,选择9~11月龄、1岁~组、3岁~组、5岁~组、7岁~组、10岁~组、15岁~组和20岁及以上人群采集血液标本,采用酶联免疫吸附试验(ELISA)检测麻疹IgG抗体。结果 1 200名监测对象,麻疹抗体阳性率为87.00%、保护率45.91%和抗体几何平均浓度(GMC)944.60 mIU/ml;8个年龄组人群麻疹抗体阳性率以5岁~组最低(81.33%)、1岁~组最高(92.67%),不同年龄组差异无统计学意义(χ^2=12.850,P>0.05),保护率以9~11月龄最高(71.33%)、15岁~组最低(29.33%);男性抗体阳性率(87.67%)高于女性(86.36%),不同性别差异无统计学意义(χ^2=0.453,P>0.05);拜城县麻疹抗体阳性率最低(76.67%)、库车县最高(91.67%),各县差异有统计学意义(χ^2=30.312,P<0.05)。结论拜城县和部分年龄组人群麻疹抗体阳性率较低,健康人群麻疹抗体水平较低是近几年阿克苏地区麻疹发病的一个重要原因;应加强重点地区和重点人群疫苗预防接种管理,提高接种率。
Objective To understand the antibody level of measles in healthy peope in Akesu prefecture, so as to provide scientific reference for formulating immunization strategy of measles. Methods Totally 1 200 healthy people were selected as the age groups of 9-11 months, 1 year-, 3 years-, 5 years-, 7 years-, 10 years-, 15 years-, 20 years and elder with stratified cluster sampling method from 5 counties of Akesu prefecture. Blood samples were collected for testing antibody IgG to measles with enzyme linked immunosorbent assay(ELISA). Results Out of the 1 200 subjects, the positivity rate of measles antibody was 87.00%, with the protective rate of 45.91% and the geometric mean concentrations(GMC) of 944.60 mIU/ml. Out of the 8 age groups, the positivity rate of measles antibody was the lowest(81.33%) at age group of 5 years-,and the highest(92.67%) at age group of 1 year-, without statistical difference(χ^2=12.850, P>0.05). The protection rate was the highest(71.33%) at age group of 9-11 months, and the lowest(29.33%) at age group of 15 years-. The positive rate of measles antibody was higher in the male(87.67%) than in the female(86.36%) without statistical difference(χ^2=0.453, P>0.05). The positive rate of measles antibody was the lowest in Baicheng county(76.67%), and the highest in Kuche county(91.67%). There was statistical difference(χ^2=30.312, P<0.05) in the positive rates of measles antibody among counties. Conclusions The positive rate of measles antibody is low in some age groups and in Baicheng county. The antibody level of measles is low in healthy people, and may be one of the important factors for measles epidemic in Akesu prefecture in recent years. The vaccination management should be strengthened in the key population in the prefecture.
