摘要
目的探讨丁苯酞注射液联合舒洛地特治疗急性进展性脑梗死的临床效果及其对患者炎性因子水平和血液流变学的影响。方法选择2017年12月至2019年12月阜阳市第二人民医院收治的78例急性进展性脑梗死患者为研究对象,依据随机数字表法将其分为观察组和对照组,每组各39例。对照组患者接受舒洛地特注射液治疗,观察组患者接受舒洛地特注射液联合丁苯酞注射液治疗。两组患者疗程均为14 d。比较两组患者治疗效果、治疗前后改良Rankin量表(modified Rankin scale,mRS)评分和美国国家卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分、炎性因子水平、血液流变学指标。结果观察组患者治疗总有效率显著高于对照组(P<0.05)。两组患者治疗后mRS评分和NIHSS评分、血清白介素-6和C反应蛋白水平、全血低切黏度、血浆黏度和全血高切黏度均显著低于本组治疗前(均P<0.05),且观察组患者上述指标水平均显著低于对照组(均P<0.05)。结论丁苯酞注射液联合舒洛地特治疗急性进展性脑梗死疗效显著,可减轻炎性反应,改善患者血液流变学。
Objective To investigate the therapeutic effect of butylphthalide injection combined with sulodexide in the treatment of acute progressive cerebral infarction and its influence on inflammatory factors and hemorheology.Method A total of 78 patients with acute progressive cerebral infarction admitted in Fuyang Second People's Hospital from December 2017 to December 2019 were randomly divided into observation group and control group,with 39 cases in each group.Patients in control group were treated with sulodexide,patients in observation group were treated with sulodexide combined with butylphthalide and sodium chloride injection.The course of treatment in both groups was 14 days.The therapeutic effect,modified Rankin scale(mRS)score,National Institute of Health stroke scale(NIHSS)score,inflammatory factors levels and hemorheology indexes of the two groups were compared before and after treatment.Result The total effective rate of observation group was significantly higher than that of control group(P<0.05).After treatment,the mRS score,NIHSS score,serum interleukin-6 and C reactive protein level,whole blood viscosity low shear,plasma viscosity and whole blood viscosity high shear of the two groups were significantly lower than those before treatment(all P<0.05),and the above indexes levels in observation group were significantly lower than those in control group(all P<0.05).Conclusion Butylphthalide injection combined with sulodexide has obvious curative effect on acute progressive cerebral infarction,which can reduce inflammatory reaction and improve hemorheology of patients.
