摘要
目的:探讨达芬奇机器人辅助直肠癌经自然腔道取标本手术的临床应用价值。方法:回顾性分析2017-06-01~2019-03-31我科行机器人辅助直肠癌经自然腔道取标本手术26例的临床资料。结果:26例患者均顺利完成手术,平均手术时间195 min(包括装机时间20 min),平均术中出血量30 ml,术后平均胃肠功能恢复时间32 h,术后平均住院时间5 d,吻合口瘘1例;无输尿管、尿道、肠管、血管损伤发生。病理检查标本切缘均无肿瘤细胞残留,平均清扫淋巴结22枚。结论:机器人辅助直肠癌经自然腔道取标本手术安全可行,术后并发症少且无腹部切口,手术创伤小、恢复快,住院时间短,值得推广应用。
Objective:To explore the clinical value of Da Vinci robot-assisted anterior resection rectal cancer with natural orifice specimen extraction surgery(NOSES).Methods:The clinical data of 26 cases who underwent robot-assisted resection of rectal cancer with NOSES from Jun 2017 to March 2019 were retrospectively analyzed.Results:The operation was successfully performed in 26 cases,the average operative time was 195 min(included installation time 20 min),the average blood loss was 30 ml,the average recovery time of postoperative bowel function was 32 h,the average hospital stay after surgery was 5 d,1 cases was anastomotic leakage.No damage to ureter,urethra,intestine or blood vessel occurred.All patients were found with non-cancer cell infiltration,harvested lymph node number was 22.Conclusion:Robot-assisted anterior resection rectal cancer with NOSES is an effective and safe treatment.It has the advantages of no abdominal incision,minimal incision,and rapid recovery,which is worthy of clinically widely application.
