摘要
巨细胞病毒(cytomegalovirus,CMV)感染在免疫功能严重缺陷的艾滋病(acquired immunodeficiency syndrome,AIDS)患者中相当普遍,可累及全身多个器官系统如眼部、肺部、消化系统以及神经系统等,表现为不同类型的巨细胞病毒病(cytomegalovirus disease,CMVD),播散性感染亦相当常见,具有高致残性和高致死性的特点.与免疫正常人群感染CMV后多呈隐性感染不同,艾滋病合并巨细胞病毒病(AIDS/CMVD)在流行病学、发病机制、临床特征、诊断与鉴别诊断、治疗和预防等方面均具有一定特殊性,且目前尚缺乏系统的指南性文献可供参考.本共识在综合现有文献与课题研究成果基础上,就AIDS/CMVD的流行情况、发病机制、临床特征、诊断、治疗和预防等内容进行详细阐述,以期为该疾病临床诊疗提供参考.
Cytomegalovirus(CMV)infections are common in acquired immunodeficiency syndrome(AIDS)patients whose immune system becomes functionally impaired.CMV may involve multiple organ systems,viz.the ocular pathways,the pulmonary organs,the digestive system and the central and peripheral nervous system,producing different clinical manifestations of cytomegalovirus disease(CMVD).Disseminated CMV infection is also quite common.CMVD has the potential to inflict significant morbidity and mortality in susceptible populations.Unlike CMV infections in individuals with a normal immune system,acquired immunodeficiency syndrome/cytomegalovirus disease(AIDS/CMVD)has disease-specific features in its epidemiology,pathogenesis,clinical manifestations,diagnosis and differential diagnosis,treatment,and prevention.However,currently no systematic guideline for AIDS/CMVD is available for reference.This consensus document discusses the epidemiology,pathogenesis,clinical manifestations,diagnosis,treatment and prevention of AIDS/CMVD based on current data derived from existing literature and research results,and aims to provide a reference for the clinical diagnosis and treatment of AIDS/CMVD.
