摘要
探讨目前新型冠状病毒肺炎(COVID-19)中医诊疗过程中的主要分歧,明确其病因病机及治疗原则。方法:通过理论研究并结合COVID-19患者临床资料,对诊疗过程中存在的主要分歧进行梳理,明确COVID-19的中医学范畴,对病机中的"湿"及病性的寒热属性进行分析,从而明确其病因病机及治疗原则。结果:COVID-19属于木疫,其性为风热。其中的"湿"乃太阴内伤,湿饮停聚,外加风热疫毒,肝木乘克脾土,脾虚运化无权,湿邪内生所致。治疗过程中应三焦辨证与卫气营血辨证相结合进行分期辨证论治,清热与化湿兼顾,尤其注意固护中焦脾胃。结论:中医药在对COVID-19的诊疗过程中,应将五运六气理论与传统辨证论治和参,抓住疾病的根本病机,明确其演变规律,灵活运用各种辨证论治方法,结合患者体质选方用药。
To explore the main differences in the diagnosis and treatment of COVID-19 in TCMandclarify its etiology,pathogenesis and treatment principles.Methods:Through theoretical research and combined with clinical data of COVID-19 patients,the main differences in the diagnosis and treatment process were combed,the TCM category of COVID-19 was clarified,the"dampness"in the pathogenesis and the cold and heat nature of the disease were analyzed,so as to clarify the etiology,pathogenesis and treatment principles.Results:COVID-19 belongs to wood plague,and its nature is wind-heat.Among them,"dampness"is caused by the internal injury of Taiyin.The accumulation of the dampness and the attack of wind-heat would make theliver wood restrain the spleen earth.As a result,the spleen deficiency would lead to dysfunction of transformationand the internal dampness would be formed.In the course of treatment,it is necessary to combine the syndrome differentiation of Sanjiao with that of Wei-Qi-Ying-Blood in order to treat by stages,to give consideration to both heat-clearing and dampness-removing,especially to strengthen the spleen and stomach in Zhongjiao.Conclusion:In the process of diagnosis and treatment of COVID-19,TCM should combine the theory of five Yun and six Qi with traditional syndrome differentiation and treatment,grasp the basic pathogenesis of the disease,make clear its evolution law,flexibly use various syndrome differentiation and treatment methods,and select the prescription based on the patients’constitution.
