摘要
肌肉减少症属于中医学"痿(病)证"范畴,以精髓、气血亏虚以致筋脉弛缓,软弱无力、不能随意运动为特征的一种疾病。临床表现为肢体筋脉弛缓,软弱无力,活动不利,甚则肌肉萎缩,不能持重或不能久立、久行,可伴有肢体麻木、疼痛或拘急痉挛,严重可见排尿困难、呼吸困难、吞咽无力等。中医治疗总则为补肾填精以养先天之本、健脾和胃益气血以补后天,兼以祛湿热,化痰饮、消瘀血。该病辨证论治:肺热津伤证治以清热润燥,滋养筋脉,常用清燥救肺汤加减;气滞血瘀证治以行气活血,通络养营,药用柴胡疏肝散或圣愈汤合血府逐瘀汤加减;脾胃虚弱证治以健脾养胃,益气通脉,方用补中益气汤合参苓白术散加减;肝肾亏虚证治以滋补肝肾,强筋健骨,方用虎潜丸加减。临证还可用针灸推拿和导引法治疗,效果肯定。
Musculopenia belongs to the category of"flaccidity(disease)syndrome"in traditional Chinese medicine.It is characterized by flaccid tendons and arteries due to deficiency of essence and qi and blood,weakness and inability to voluntary movement.The clinical manifestations are flaccid limbs,weak and weak muscles,unfavorable activities,even muscular atrophy,inability to hold or stand for a long time,accompanied by numbness,pain or spasm of limbs,and severe dysuria,dyspnea,dysphagia,etc.The general principles of TCM treatment are to tonify the kidney and replenish essence to nourish the innate foundation,to strengthen the spleen and stomach,to replenish qi and blood to replenish the acquired,and to eliminate damp heat,phlegm and blood stasis.The syndrome differentiation and treatment of the disease are as follows:The treatment of lung heat and fluid injury is to clear away heat and moisten dryness and nourish the tendons and arteries,and Qingzao Jiufei Decoction is commonly used.Qi stagnation and blood stasis syndrome is treated by activating qi and blood circulation,dredging collaterals and nourishing yin,and is modified by Chaihu Shugan Powder or Shengyu Decoction combined with Xuefu Zhuyu Decoction.The spleen and stomach weakness syndrome is treated by strengthening spleen and nourishing stomach,invigorating qi and dredging pulse,and the formula is modified by Buzhong Yiqi Decoction and Shenling Baizhu Powder.The deficiency of liver and kidney is treated by nourishing liver and kidney,strengthening tendons and bones,and the formula is modified by Huqian Pill.Clinical manifestations can also be treated with acupuncture,massage and guidance,with definite results.
