摘要
目的研究乙型肝炎病毒(hepatitis B virus,HBV)感染孕妇人工流产术后HBV DNA定量与其自身肠道微生物的相关性。方法选取2016年2月至2018年2月深圳市龙岗区第三人民医院诊治的50例孕早期HBV感染孕妇作为研究对象。将这50例孕妇设为试验组,同期进行孕检的孕早期无HBV感染孕妇50例作为对照组。两组孕妇均接受人工流产术,分别进行胚胎组织的制备以及孕妇肠道排粪物的获取,通过荧光定量PCR检测两组胚胎HBV DNA定量,比较两组孕妇肠道微生物(主要囊括粪肠球菌、双歧杆菌、乳酸杆菌、白色念珠菌)计数水平,并作相关性分析。结果试验组的胚胎HBV DNA检出率为66.00%,高于对照组的0.00%,差异具有统计学意义(P<0.05)。试验组的粪肠球菌、白色念珠菌计数均高于对照组,而双歧杆菌、乳酸杆菌计数均低于对照组,差异均具有统计学意义(均P<0.05)。经Pearson相关性分析可得:HBV感染孕妇的HBV DNA定量与其肠道内粪肠球菌、白色念珠菌计数呈正相关,差异具有统计学意义(r=0.683、0.619;P=0.000、0.000),与双歧杆菌、乳酸杆菌计数呈负相关,差异具有统计学意义(r=-0.607、-0.603;P=0.000、0.000)。结论孕妇的HBV感染与其肠道微生物的平衡状态密切相关,值得临床重视。
Objective To investigate the correlation between HBV DNA quantification of hepatitis B virus(HBV)infected after induced abortion of pregnant women and their intestinal microorganisms.Methods 50 HBV-infected pregnant women in early pregnancy who received pregnancy examination in the Third People′s Hospital of Longgang District from February 2016 to February 2018 were selected and recorded as the experimental group.Another 50 HBV-free pregnant women who underwent pregnancy examination in the Third People′s Hospital of Longgang District during the same period were selected as the control group.All the pregnant women in the two groups received artificial abortion,embryonic tissues preparation and collection of intestinal tract defecation.The HBV DNA quantification was detected by fluorescence quantitative PCR and intestinal microorganisms(Enterococcus faecalis,Bifidobacterium,Lactobacillus,Candida albicans)count in the two groups were compared for correlation analysis.Results The detection rate of fetal HBV DNA in the experimental group was 66.00%,higher than that in the control group(0.00%),with statistically significant differences(all P<0.05).The counts of Enterococcus faecalis and Candida albicans in the experimental group were higher than those in the control group,while the counts of Bifidobacterium and Lactobacillus were lower than those in the control group,with statistically significant differences(all P<0.05).According to Pearson correlation analysis,HBV DNA quantification of HBV infected pregnant women was positively correlated with counts of Enterococcus faecalis and Candida albicans(r=0.683,0.619;P=0.000,0.000),and negatively correlated with counts of Bifidobacteria and Lactobacillus(r=-0.607,-0.603;P=0.000,0.000).Conclusions HBV infection in pregnant women is closely related to the balance of their intestinal microorganisms,which is worthy of clinical attention.
