摘要
目的检测免疫球蛋白A(IgA)肾病患儿血清S100钙结合蛋白A8/A9复合物(S100A8/A9)水平,并分析其与患儿病变活动性的相关性。方法2016年8月-2018年8月郑州儿童医院肾脏风湿科收治的IgA肾病患儿90例为IgA肾病组,同时选取同期体检健康者92人为对照组。采用酶联免疫吸附(ELISA)法测定患者血清S100A8/A9水平;采用多功能全自动生化分析仪进行肾功能指标测定。采用Pearson法分析血清S100A8/A9与肾功能指标及肾脏病理指数的相关性,以及Cox回归分析影响IgA肾病患儿预后的危险因素。结果IgA肾病组患儿血清S100A8/A9水平(3.76±1.65)μg/mL高于对照组(1.28±0.49)μg/mL,差异有统计学意义(P<0.05);IgA肾病组患儿血肌酐(Scr)、24 h尿蛋白定量(24 h UTP)水平及肾脏总活动性指数(AI)积分均高于对照组,肾小球滤过率(GFR)水平低于对照组,差异有统计学意义(P<0.05);IgA肾病患儿血清S100A8/A9水平与Scr、24 h UTP水平及AI积分均呈正相关(r=0.535、0.612、0.569),与GFR水平呈负相关(r=-0.583),差异有统计学意义(P<0.05)。多因素分析显示,血清S100A8/A9高水平是影响IgA肾病患儿不良预后的独立危险因素(HR=2.391,95%CI:1.327~4.308,P<0.05)。结论S100A8/A9在IgA肾病患儿血清中呈高表达,与IgA肾病患儿病变活动性密切相关,可能作为判断预后的生物指标。
Objective To detect the level of serum S100 calcium binding protein A8/A9 complex(S100 A8/A9)in children with immunoglobulin A nephropathy(IgA nephropathy),and analyze its correlation with the activity of the disease.Methods 90 children with IgA nephropathy admitted to department of nephrology and rheumatology dept of Zhengzhou Children′s Hospital from August 2016 to August 2018 were selected as IgA nephropathy group,and 92 healthy people were selected as control group.The level of serum S100 A8/A9 was measured by enzyme-linked immunosorbent assay(ELISA)and renal function was measured by multi-functional automatic biochemical analyzer.Pearson method was used to analyze the correlation between serum S100 A8/A9,renal function index and renal pathological index,and Cox regression analysis was used to analyze the risk factors affecting the prognosis of IgA nephropathy.Results The level of serum S100 A8/A9 in IgA nephropathy group(3.76±1.65)μg/mL was significantly higher than that in control group(1.28±0.49)μg/mL(P<0.05).The levels of serum creatinine(Scr),24 h urinary protein(24 h UTP)and total renal activity index(AI)score in IgA nephropathy group were higher than those in control group,and the glomerular filtration rate(GFR)level was lower than that in control group,the differences were significant(P<0.05).Serum S100 A8/A9 level in children with IgA nephropathy was positively correlated with Scr,24 h UTP levels and AI score(r=0.535,0.612,0.569),and negatively correlated with GFR levels(r=-0.583),with statistical significances(P<0.05).Multivariate analysis showed that the high level of serum S100 A8/A9 was an independent risk factor for the poor prognosis of children with IgA nephropathy(HR=2.391,95%CI:1.327-4.308,P<0.05).Conclusion S100 A8/A9 was highly expressed in serum of children with IgA nephropathy,which was closely related to the activity of IgA nephropathy,and might be used as a biomarker for judging prognosis.
