摘要
目的探讨宫颈癌患者BMI对放疗摆位误差的影响.方法将应用Varian Turebeam直线加速器行放射治疗的80例宫颈癌患者按体质量指数分为对照组(31例)和超重组(49例),统计校正前后两组患者在左右位、头脚位、前后位三个轴向上的摆位误差;计算临床靶区到计划靶区的外扩边界,并统计两组患者的摆位修正率.结果校正前,超重组左右位、头脚位以及前后位三个轴向上的摆位误差均明显大于对照组(P<0.05);校正后,两组摆位误差均显著减小(P<0.05),两组间比较差异无统计学意义(P>0.05).超重组摆位外放边界在左右位、头脚位以及前后位三个轴向上较对照组分别大1.07 mm、2.01 mm及1.59 mm.对照组摆位修正率主要集中在<3.0 mm及≤5.0 mm,超重组在各范围内无明显集中现象.结论宫颈癌放疗患者的摆位误差与体质量指数关系密切,对体质量指数较大的患者应进行影像引导放疗.
Objective To observe the relationship between setup error and body mass index(BMI)in patients with cervical cancer undergoing radiotherapy.Methods 80 patients with cervical cancer who underwent radiotherapy by Varian Turebeam linear accelerator were divided into control group(31 cases)and overweight group(49 cases).The differences in setup errors in three axial directions[lateral(LAT),head pin(LNG),dorsoventral(VRT)]in both groups before and after correction were observed and statistically analyzed.The PTV margin(MPTV)from CTV to PTV was calculated.The setup correction rates in both groups were statistically analyzed and recorded.Results The setup errors in LAT,LNG and VRT axial directions of overweight group were significantly larger than those of control group(P<0.05).After correction,setup errors were significantly reduced in both groups(P<0.05),and there was no significant difference between the two groups(P>0.05).The MPTV levels in LAT,LNG and VRT axial directions of overweight group were 1.07 mm,2.01 mm and 1.59 mm larger than those of control group.The setup correction rates in control group were mainly concentrated at<3.0 mm and≤5.0 mm.There was no obvious concentration of setup correction rate at each range in overweight group.Conclusions The setup error of patients with cervical cancer radiotherapy is closely related to body mass index.Radiotherapy guided by imaging should be performed for patients with a larger BMI.
