厄贝沙坦联合氨氯地平对老年原发性高血压合并T2DM患者血清Hcy及BNP水平的影响 被引量:12

摘要 目的探讨厄贝沙坦联合氨氯地平对老年原发性高血压合并2型糖尿病患者血清同型半胱氨酸及脑钠肽水平的影响.方法将120例老年原发性高血压合并2型糖尿病患者按随机数字表法分为甲、乙、丙3组,每组40例,甲组予以氨氯地平联合厄贝沙坦治疗,乙组予以厄贝沙坦治疗,丙组予以氨氯地平治疗.于治疗前后采用酶联免疫吸附法测定血清同型半胱氨酸水平,采用快速荧光免疫法测定血清脑钠肽水平,采用血浆葡萄糖氧化酶法检测空腹血糖和餐后2 h血糖,同时测量患者静息状态血压.结果治疗后3组患者血压、血清同型半胱氨酸、血清脑钠肽、空腹血糖及餐后2 h血糖水平均较治疗前显著降低(P<0.05或0.01),甲组显著低于乙组、丙组(P<0.01).结论厄贝沙坦联合氨氯地平治疗老年原发性高血压合并2型糖尿病能有效控制患者血压及血糖水平,降低血清同型半胱氨酸及脑钠肽水平,优于单用氨氯地平或厄贝沙坦治疗. Objective To investigate the effect of irbesartan and amlodipine on the levels of homocysteine(Hcy)and brain natriuretic peptide(BNP)in essential hypertensive elderly patients with type 2 diabetes mellitus(T2DM).Methods 120 cases of elderly patients with essential hypertension with T2DM were were randomly divided into three groups according to random number table method,40 cases each.Group A was given amlodipine combined with irbesartan,group B was treated with irbesartan alone,group C was treated with amlodipine alone.Serum homocysteine was measured by enzyme-linked immunosorbent assay before and after treatment,and serum brain natriuretic peptide levels were measured by rapid fluorescent immunoassay.Plasma glucose oxidase method was used to measure fasting blood glucose and 2 h postprandial blood glucose.At the same time,the patien'ts resting blood pressure was measured.Results After treatment,the blood pressure,serum Hcy,serum BNP,fasting blood glucose,and 2 h postprandial blood glucose levels in the three groups were significantly lower than before treatment(P<0.05 or 0.01).Group A was significantly lower than group B and group C(P<0.01).Conclusions Irbesartan combined with amlodipine in the treatment of elderly patients with essential hypertension combined with type 2 diabetes can effectively control blood pressure and blood sugar levels,reduce serum homocysteine and brain natriuretic peptide levels,which is better than that of amlodipine or irbesartan alone.
