摘要
目的探讨NRS-2002在耐多药肺结核患者营养风险筛查中的临床应用价值。方法前瞻性选取我院2016年1月~2019年10月收治的耐多药肺结核患者,采用NRS-2002进行营养风险筛查,同时测定上臂肌围、肱三头肌皮褶厚度、体质量指数(BMI)、血浆白蛋白(ALB)、血红蛋白(HB)、前白蛋白(PA)等营养指标,分析NRS-2002评分与各项营养指标的相关性。结果共有66例耐多药肺结核患者纳入本次研究,具有营养风险的有50例,营养风险发生率为75.76%。与无营养风险患者相比,有营养风险患者年龄明显偏大,BMI、ALB、HB和PA明显下降,差异均具有统计学意义(P<0.05)。相关性分析表明,NRS-2002评分与年龄呈明显正相关(P<0.05),与BMI、ALB、HB和PA呈明显负相关(P<0.05)。结论耐多药肺结核患者具有较高的营养风险发生率。使用NRS-2002筛查耐多药肺结核患者的营养风险简单、有效,有一定的临床应用价值。
Objective To explore the clinical application value of NRS-2002 in nutrition risk screening for patients with multidrug-resistant tuberculosis.Methods Patients with multidrug-resistant tuberculosis treated in our hospital from January 2016 to October 2019 were prospectively selected and screened for nutritional risk using NRS-2002.At the same time,upper arm muscle circumference,triceps skin fold thickness,body weight index(BMI),plasma albumin(ALB),hemoglobin(HB),and prealbumin(PA)were simultaneously determined.The correlation between the NRS-2002 score and various nutritional indicators was analyzed.Results A total of 66 patients with multidrug-resistant tuberculosis were included in this study.50 patients had nutritional risk,and the incidence of nutritional risk was 75.76%.Compared with patients without nutritional risk,patients with nutritional risk were significantly older,and had significantly decreased BMI,ALB,HB,and PA,and the differences were statistically significant(P<0.05).Correlation analysis showed that the NRS-2002 score was significantly positively correlated with age(P<0.05),and significantly negatively correlated with BMI,ALB,HB,and PA(P<0.05).Conclusion Patients with MDR-TB have a high incidence of nutritional risk.Using NRS-2002 to screen the nutritional risk of patients with multidrug-resistant tuberculosis is simple,effective,and has certain clinical application value.
