摘要
目的观察评价分析自由体位(FP)配合分娩球(BB)对初产妇足月妊娠分娩结局的影响效果。方法选择2017年10月至2018年12月期间本院产科收治的即将分娩的初产妇98例,随机对照试验(RCT)分为两组,各49例。对照组给予传统卧位或半卧位待产及分娩;观察组给予自由体位(FP)配合分娩球(BB)助产方式,并比较两组的干预效果。结果观察组第一产程、第二产程、第三产程、总产程、产后2 h出血量分别为(247.26±96.15)min、(31.29±14.56)min、(5.19±3.07)min、(283.74±113.78)min、(189.06±43.27)ml,均低于对照组,差异均有统计学意义(均P<0.05);观察组自然分娩率为81.63%,高于对照组42.86%,剖宫产率、产钳助产率分别为8.16%、10.20%,均低于对照组26.53%、30.61%,差异均有统计学意义(均P<0.05);两组新生儿Apgar评分比较,差异无统计学意义(P>0.05);观察组疼痛分级0~3级缓解率均明显优于对照组,差异均有统计学意义(均P<0.05);观察组VAS评分为(2.08±0.29)分,低于对照组,差异有统计学意义(P<0.05)。结论自由体位(FP)配合分娩球(BB)对初产妇足月妊娠(TP)分娩结局具有理想的积极效果,有效缩短了第一、二、三产程时间、总产程时间,降低了产后2 h出血量,减少产妇的痛苦,提高了自然分娩率,大大的改善产妇预后。
Objective To observe,evaluate,and analyze the effect of free posture(FP)combined with birth ball(BB)on the outcomes of full-term pregnancy and delivery in primiparae.Methods A total of 98 primiparae who were about to give birth at our department from October,2017 to December,2018 were selected,and divided into a control group and an observation group by randomized controlled trial(RCT),with 49 in each group.The control group took the traditional supine position or the semi-recumbent position for delivery.The observation group took a FP and delivered assisted by BB.The intervention effects of the two groups were compared.Results The first,second,and third stages of labor,the total duration of labor,and the 2 h postpartum hemorrhage in the observation group were(247.26±96.15)min,(31.29±14.56)min,(5.19±3.07)min,(283.74±113.78)min,and(189.06±43.27)ml,which were significantly lower than those in the control group(all P<0.05).The natural delivery rate was significantly higher in the observation group than in the control group(81.63%vs.42.86%;P<0.05).The cesarean section rate and forceps assist rate were 8.16%and 10.20%in the observation group,which were significantly lower than those in the control group(both P<0.05).There was no statistical difference in Apgar scores between the two groups(P>0.05).The pain relief rates in grades 0 to 3 were significantly better in the observation group than in the control group(all P<0.05).The VAS score was(2.08±0.29)in the observation group,which was lower than that in the control group(P<0.05).Conclusion FP combined with BB has ideal positive effects on the delivery outcomes of full-term pregnancy in primiparae,effectively shortens the first,second,and third stages of labor as well as the total labor time,reduces the bleeding volume 2 hours after delivery and maternal pain,improves the rate of natural delivery,and greatly improves the women’s prognosis.
