摘要
目的以某地市级医院农村订单定向全科住院医师为调查对象,考察全科住院医师规范化培训(住培)工作现状、存在的问题与困难,为进一步改进全科住培模式、提高培训质量提供参考。方法选取该基地招收的农村订单定向全科住院医师152名,采用自行设计的问卷进行调查,问卷内容包括住院医师基本情况、对全科住培工作的认识、对临床实践基地和基层实践基地培训实施满意度等。发放问卷152份,回收有效问卷152份,有效率100%。结果 152名住院医师中少数民族占43.42%,86.18%的住院医师来自一级医疗机构。住院医师对住培各项工作满意度较高,其中82.89%的住院医师肯定目前住培相关政策,认为通过培训得到提高的能力依次为临床实践及专业技能、自主学习能力和人际交往能力,但在师资队伍建设、教学活动及基层实践环节等方面有待进一步加强。结论建议通过加大全科师资培养力度和建立动态管理机制、规范教学活动和考核监管、加强临床和基层实践基地联动等重点环节的精细化管理,以岗位胜任力为导向培养合格全科医师。
Objective Our study aimed to investigate the current situations, existing problems and difficulties in standardized residency training of general medicine to provide reference for further improving improvement. The study was conducted in a prefectural hospital. Methods 152 rural-oriented residents of general medicine in the standardized residency training were involved in the study with self-designed questionnaires. The questionnaire included information on basic background, residents’ understandings about standardized residency training of general medicine, and if they were satisfied with the training they received in both hospitals and communities. 152 valid questionnaires were collected, with the effective recover rate of 100%. Results Among the 152 trainees, 43.42% belonged to the minority nationality, and 86.18% were from community hospitals. Most residents were satisfied with their training process, and 82.89% of them were supportive for the existing policies. Their clinical skills, professional knowledge,self-learning abilities and interpersonal skills were significantly improved through the training. However, they suggested that the teaching staff construction, course and community practice arrangement should be further strengthened. Conclusion It is recommended to strengthen the training of general practice teachers, establish a dynamic management mechanism, standardize teaching activities and assessment supervision, strengthen the management of the key links like the linkage of clinical and community practice bases, so as to improve the teaching ability of community practice bases. Competency-based model is the key to train qualified general practitioners.
