缺血性脑卒中患者单个核细胞TLR3、IRF-3、IFN-γmRNA表达及临床意义 被引量:16

Expression of TLR3,IRF⁃3,IFN⁃γmRNA in mononuclear cells of patients with ischemic cerebral stroke and its clinical significance

摘要 目的探讨缺血性脑卒中患者单个核细胞Toll样受体3(TLR3)以及干扰素调节因子-3(IRF-3)、干扰素-γ(IFN-γ)表达及意义。方法选取2016年2月至2019年7月治疗的缺血性脑卒中患者108例(观察组),其中轻度、中度和重度患者分别30例、42例和36例;预后良好66例,不良42例;同时选取健康志愿者80例作为对照组,采用实时荧光定量PCR法检测单个核细胞TLR3、IRF-3、IFN-γmRNA表达。结果观察组TLR3、IRF-3和IFN-γmRNA相对表达量分别为(7.211±2.121)、(5.801±1.821)和(6.270±1.533),明显高于对照组(P<0.05);重度患者脑梗死体积、TLR3、IRF-3和IFN-γmRNA相对表达量明显高于轻度和中度患者(P<0.05);预后不良患者脑梗死体积、TLR3、IRF-3和IFN-γmRNA相对表达量明显高于预后良好患者(P<0.05);观察组治疗后TLR3、IRF-3和IFN-γmRNA相对表达量较治疗前降低(P<0.05);TLR3 mRNA表达与IRF-3 mRNA、IFN-γmRNA、NIHSS评分和脑梗死体积呈正相关(r=0.561、0.508、0.400和0.311,P<0.05);IRF-3mRNA与IFN-γmRNA呈正相关(r=0.302,P<0.05)。结论缺血性脑卒中患者TLR3、IRF-3和IFN-γmRNA表达明显升高,其中TLR3 mRNA与患者病情程度、脑梗死体积和预后有一定关系。 Objective To investigate the expression and significance ofmononuclear cell Toll like receptor 3(TLR3),interferon regulatory factor-3(IRF-3),interferon γ(IFN-γ) in patients with cerebral ischemic stroke.Methods A total of 108 patients with ischemic stroke(observation group) treated from February 2016 to July 2019 were selected,of which 30,42 and 36 patients were mild,moderate and severe condition.66 Patients had good prognosis,and 42 poor.80 healthy volunteers were selected as the control group,and the expression mRNA of TLR3,IRF-3,IFN-y mRNA in mononuclear cells was detected by real-time fluorescent quantitative PCR.Results The levels of TLR3,IRF-3 and IFN-γ mRNA in observation group were(7.211±2.121),(5.801±1.821) and(6.270±1.533),which were significantly higher than the control group(P<0.05).The cerebral infarction volume,TLR3,IRF-3 and IFN-γ mRNA levels in severe patients were higher than that in mild and moderate patients(P<0.05),those in patients with the poor prognosis were significantly higher than that in patients with the good prognosis patients(P<0.05).The levels of TLR3,IRF-3 and IFN-γmRNA in the observation group after treatment were lower than those before treatment(P<0.05).The level of TLR3 mRNA was positively related with IRF-3 mRNA,IFN-γ mRNA,NIHSS score and cerebral infarction volume(r=0.561,0.508,0.400 and 0.311,P<0.05).The IRF-3 mRNA level was positively correlated with the IFN-γ mRNA level(r=0.302,P<0.05).Conclusion The levels of TLR3,IRF-3 and IFN-γ mRNA in cerebral ischemic stroke patients increased significantly.The TLR3 mRNA level is related to the patient’s condition,the volume of cerebral infarction and the prognosis.
