摘要
目的探讨加速康复外科(ERAS)护理理念应用于妇科恶性肿瘤微创手术患者围手术期中的临床效果。方法采用便利抽样法收集2018年8月-2019年5月在同济大学附属第一妇婴保健院行妇科恶性肿瘤微创手术的患者60例,根据随机数字表法分为非ERAS组与ERAS组,每组30例,非ERAS组患者行常规护理,ERAS组患者实施加速康复外科护理。比较两组患者术后首次进食时间、术后3 d下床活动时间、术后3 d活动量、肛门排气时间、排便时间、住院时间、住院总费用、并发症、疼痛评分和满意度评分。结果ERAS组患者术后3 d下床活动时间、活动总量均多于非ERAS组,术后3 d总补液量少于非ERAS组,肛门排气时间、排便时间早于非ERAS组,术后第2天疼痛评分优于非ERAS组,满意度高于非ERAS组患者,差异均有统计学意义(P<0.05)。结论ERAS护理理念应用于妇科恶性肿瘤微创手术患者,提升患者就医体验,能够加快患者术后康复进程、缩短恢复时间,值得临床推广应用。
Objective To explore the clinical effects of enhanced recovery after surgery(ERAS)nursing concept in perioperative period of patients with gynecological malignant tumors undergoing minimally invasive surgery.Methods A total of 60 patients with gynecological malignant tumors who underwent minimally invasive surgery in Shanghai First Maternity and Infant Hospital from August 2018 to May 2019 were selected by convenience sampling method.According to the random number table,they were divided into the non-ERAS group and ERAS group,with 30 cases in each group.Patients in the non-ERAS group were given routine nursing while patients in ERAS group were given enhanced recovery after surgery nursing.The first feeding time after operation,the out of bed activity time on 3 days after operation,activity amount of 3 days after operation,anal exsufflation time,defecation time,length of hospital stay,total hospitalization expenditures,complications,pain score and satisfaction score were compared between the two groups.Results The out of bed activity time on 3 days after operation and activity amount of 3 days after operation of patients in ERAS group were both more than those of the non-ERAS group,volume of fluid input on 3 days after operation was less than that of the non-ERAS group,anal exsufflation time and defecation time were earlier than those of the non-ERAS group,the pain score on the second day after the operation was better than that of the non-ERAS group and satisfaction degree was higher than that of the non-ERAS group.The differences were statistically significant(P<0.05).Conclusions Application of enhanced recovery after surgery nursing in minimally invasive surgery for gynecologic malignant tumors can improve medical experience of patients,accelerate the postoperative recovery process,shorten the recovery time of patients and help patients to recover,which is worthy of promotion and application.
