摘要
目的探讨免疫组织化学检测在乳腺病理诊断中的应用价值。方法针对2018年2月-2019年5月该院收治的乳腺病患者中选择43例乳腺癌患者和56例乳腺良性病变患者作为该次研究对象,分别为试验组与参照组。所有患者均采用免疫组织化学检测法,观察99例患者的免疫组织化学检测结果。结果 43例乳腺癌患者中,试验组EGFR(+)、CK5/6(+)P16(+)指标的诊断准确率分别为:90.70%、95.35%、79.07%高于对照组的39.29%、46.43%、32.14%,试验组检测结果的HER-2指标的阳性率为(86.05%)明显高于参照组(33.93%),差异有统计学意义(χ^2=7.014、6.832、6.315、6.706,P<0.05)。CK5/6、EGFR、P16等指标阳性表达与乳腺癌呈正相关。在对试验组患者样本进一步检测时发现,ER、EP均为阳性、C-erbB-2同为阳性的占比27.27%,C-erbB-2同为阴性的占比72.72%。在ER、RP均为阴性的患者中,C-erbB-2同为阳性的占比80.00%,C-erbB-2同为阴性的占比20.00%。CerbB-2的表达与ER、EP成负相关。结论免疫组织化学检测可以较好地鉴别乳腺恶良性病变,在乳腺病理诊断中具有较高的应用价值,可考虑将免疫组织化学检测的CK5/6、EGFR、P16等指标作为乳腺癌诊断的重要参考。
Objective To explore the application value of immunohistochemical detection in breast pathological diagnosis. Methods For breast cancer patients admitted to the hospital from February 2018 to May 2019, 43 patients with breast cancer and 56 patients with benign breast lesions were selected as the subjects of this study, and they were used as the experimental and reference groups in this study. All patients were tested by immunohistochemistry, and the results of immunohistochemistry were observed in 99 patients. Results Among 43 breast cancer patients, the diagnostic accuracy rates of EGFR(+) and CK5/6(+) P16(+) indicators in the test group were 90.70%, 95.35%, and 79.07%higher than those in the control group, 39.29% and 46.43%, 32.14%, the positive rate of HER-2 index in the test group(86.05%) was significantly higher than the reference group(33.93%), the difference was statistically significant(χ^2=7.014, 6.832, 6.315, 6.706, P<0.05). Positive expressions of CK5/6, EGFR, P16 and other indicators were positively correlated with breast cancer. In further testing of the patient samples in the test group, it was found that ER and EP were positive, C-erbB-2 were both positive, accounting for 27.27%, and C-erbB-2 were negative, accounting for72.72%. Among patients with negative ER and RP, 80.00% were C-erbB-2 positive, and 20.00% were C-erbB-2 negative. The expression of C-erbB-2 was negatively correlated with ER and EP. Conclusion Immunohistochemical detection can better identify benign breast lesions, and has higher application value in breast pathological diagnosis.CK5/6, EGFR, P16 and other indicators can be considered as important for breast cancer diagnosis and reference.
