摘要
目的:通过Meta分析比较臂式输液港与胸式输液港相关并发症的发生率。方法:计算机检索PubMed、Embase、CINAHL、Cochrane图书馆、中国生物医学文献数据库(CBM)、万方数据库建库等至2019年1月收录的臂式输液港与胸式输液港并发症比较的研究,使用纽卡斯尔-渥太华量表评估文献质量,采用RevMan5.3进行Meta分析。结果:最终共纳入19项研究,共4203例研究对象。Meta分析结果显示,臂式输液港与胸式输液港气胸(OR=0.22,95%CI 0.05~0.88,P=0.03)及误穿动脉(OR值为0.24,95%CI 0.07~0.77,P值为0.02)的发生率差异有统计学意义;感染(OR值为0.81,95%CI 0.58~1.12,P值为0.20)、血栓(OR值为1.25,95%CI 0.64~2.45,P值为0.52)、堵管(OR值为1.13,95%CI 0.54~2.35,P值为0.75)、导管异位(OR值为1.58,95%CI 0.95~2.61,P值为0.08)、皮肤破损(OR值为0.64,95%CI 0.23~1.74,P值为0.38)、败血症(OR值为0.68,95%CI 0.27~1.70,P值为0.41)及渗液(OR值为0.88,95%CI 0.32~2.42,P值为0.80)的发生率差异均无统计学意义。结论:臂式输液港气胸和误穿动脉的发生率明显低于胸式输液港,其他并发症的发生率无明显差异,目前缺乏对并发症严重程度的研究,需更多高质量研究进一步证实。
Objective To compare the incidence of complications associated with arm and chest ports by Meta-analysis.Methods To Janurary 2019,studies published in PubMed,Embase,CINAHL,Cochrane,CBM and Wanfang regarding complication comparison between arm ports and chest ports were searched.Newcastle-Ottawa Scale was applied to evaluate the quality of studies.Review Manager 5.3 software was applied to conduct this meta-analysis.Results A total of 19 articles covering 4203 patients were included.The results showed that the incidence of pneumothorax(OR value was 0.22,95%CI0.05-0.88,P value was 0.03)and artery injury(OR value was 0.24,95%CI0.07-0.77,P value was 0.02)between arm ports group and chest ports group were statistically significant.No significant difference was observed in infection(OR value was 0.81,95%CI0.58-1.12,P value was 0.20),thrombosis(OR value was 1.25,95%CI0.64-2.45,P value was 0.52),extravasation(OR value was 1.13,95%CI0.54-2.35,P value was 0.75),catheter misplacement(OR value was 1.58,95%CI0.95-2.61,P value was 0.08),skin incision(OR value was 0.64,95%CI0.23-1.74,P value was 0.38),sepsis(OR value was 0.68,95%CI0.27-1.70,P value was 0.41)and exudate(OR value was 0.88,95%CI0.32-2.42,P value was 0.80).Conclusion The incidence of pneumothorax and artery injury in arm ports is significantly lower than in chest ports,there was no difference in other complications.More studies are needed to further confirm the advantages of arm ports.
